LOW NITROGEN PRESSURE ON "B" ACCUMULATOR

1.

Description of Event

While attempting to pressurize "B" Accumulator using normal Nitorgen supply to the accumulators, the pressure on "B" Accumulator was reduced below the allowable limits.

This event is contrary to Technical Speci-fications 3.3.A.2 and is reportable as per Technical Specifications 6.6.2.b.(2)

2.

Probable Consequences and Status of Redundant System:

Technical Specification allows for the isolation of one accumulator for a period not to exceed 4 hours4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br />.

The other two accumulators re-mained operable.

The pressure in 'B' accumulator was returned to required specification within the four hours limitation.

Therefore the health and safety of the public was not affected.

3.

Cause

The low accumulator pressure was caused by a leak and low pressure in

. the nitrogen fill header.

4.

Immediate Corrective Action

"B" accumulator was declared inoperable.

Additional nitrogen supply pressure was added to the fill system and "B" accumulator was pressurized and declared operable.

The accumulator was inoperable for 95 minutes.

5.

Subsequent Corrective Action:

Repairs were made to the leaking fittings on the fill header.

6.

Action to Prevent Recurrence:

The above corrective action will prevent recurrence.

7.
