TITLE of EVENT:

INADVERDANT OPENING of CIRCUIT BREAKER to MOV-CW-200C

1. DESCRIPTION of EVENT

With Unit 2 operating at a steady-state, power level of 100%, a routin~

check list (Cl-55) discJosed that MOV-CW-200C was de*-energized.

The failure of this MOV to close on a safeguards signal is non-conservative with respect to Technical Specification 3.14.A.1.

This eve*nt is report-able as pE':r Technical Specification 6.6.2.b_.(2).

2.

PROBABLE CONSEQUENCES and STATUS of REDUNDANT EQUIPMENT:

MOV-CW-200C received a signal to close whenever LOCA with a loss of off-site power has occurred to preserve the water in the intake canal for ESF systems.

The respective waterbox inlet MOV (MOV-CW-206C) was operable and would have closed to preserve the water in the intake canal.

Therefore, the health and safety of the public were not affected.

3.

CAUSE of the EVENT

A construction worker had hung his coat on the operating arm of the circuit breaker which opened the breaker, de-energizing MOV-CW-200C.

4.

IMMEDIATE CORRECTIVE ACTION

An operator was dispatched to check the status of the circuit breaker.

The operator removed the coat, and closed the breaker.

Then from the control room the valve was cycled to verify operability.

5.

SUBSEQUENT CORRECTIVE ACTION:

None.

6.

ACTION TAKEN to PREVENT RECURRENCE:

The construction workers working in ares were instructed not to hang anything on circuit breakers.
