2.. : REASONS.FOR VIOLATION"
- a.
The leaking safeguards building roof has been identified to the maintenance department by the operating staff.
The maint-enance report requesting the roof to be repaired was not coded for urgent action and was therefore scheduled to be accomp-lished in a
normal manner.
This unfortunately was not completed prior to: the. heavy rains of late November.

Attachment Page 2 The six inch Residual Heat Removal p1p1ng support was* first modified by the Architect-Engineer as part of the 79-14 efftirt.
This placed the support in the configuration observed by the Resident NRC Inspector.
The "as-built" effort in January, 1980 failed to note the one-half inch gap between the fixed and moveable base plates.
- c.
The several loose, missing or broken "U" bolts resulted from a design change to install new valves in the service water system.
In order to facilitate valve installation, the hangers were relaxed and not returned to their proper configuration following ~he valve installation.
