The pump motors and power cables were electrically tested and the power cable to LHSI pump 2-SI-P-lA was found wet and electrically shorted. 2.. : REASONS .FOR VIOLATION"

a. The leaking safeguards building roof has been identified to the maintenance department by the operating staff. The maint-enance report requesting the roof to be repaired was not coded for urgent action and was therefore scheduled to be accomp-lished in a normal manner. This unfortunately was not completed prior to: the . heavy rains of late November.

  • b. The six inch Residual Heat Removal p1p1ng support was* first modified by the Architect-Engineer as part of the 79-14 efftirt.

This placed the support in the configuration observed by the Resident NRC Inspector. The "as-built" effort in January, 1980 failed to note the one-half inch gap between the fixed and moveable base plates.

c. The several loose, missing or broken "U" bolts resulted from a design change to install new valves in the service water system. In order to facilitate valve installation, the hangers were relaxed and not returned to their proper configuration following ~he valve installation.
