DEFECTIVE.COIL FOR TV~MS-201C

1.

DESCRIPTION OF EVENT

With the unit at full power, the performance of PT-8. 5 (monthly CLS logic testing) revealed that TV-i1S-201C control circuit would not respond to a Train "B" actuation signal. This is contrary to Tech. Spec, 3.8.A,l and is reportable per Tech. Spec. 6.6.2.b(2).

PROBABLE CONSEQUENCES and STATUS of REDUNDA..~ EOUIPMENT:

The main steam trip valves receive a Phase 3 containment isolation signal, from both trains of CLS, to ensure double barrier isolation of lines penetrating the containment.

The redundant actuation signal, Train A, was verified to be operable, Therefore, the health and safety of the public were not affected.

CAUSE

The cause of this event was the shorting of the Train B coil due to moistur~

accumulation in an adjacent junction 1?0:k:;: -

4.

IMMEDIATE CORRECTIVE ACTION

The immediate corrective action was to verify the redundant train to be operable.

The defective coil was replaced and tested satisfactorly,

SUBSEQUENT CORRECTIVE ACTION:

The other two coils, associated with Train "B"** were replaced as a precautionary measure.

6.

ACTION TAKEN TO PREVENT RECURRENCE:

The junction box has been sealed to preclude the entrance of moisture.

7.

GENERIC IMPLICATIONS:

None.
