APPENDIX A NOTICE OF VIOLATION Missirsippi Power and Light Company License Nos. CPPR-118 Grand Julf
& CPPR-119 Based on the NRC inspection August 18-22, '980, certain of your activities were 1
apnarently not conducted in full compliance with NRC requirements as indicated below.
These items have been categoriecd as described in correspondence to you dated December 31, 1974.
A.
As required by Criterion XVll of 10 CFR 50, Appendix B, and implemented by PSAR Section 17.1.8 Audits; "... Planned and periodic audits shall be carried out to verify compliance with all aspects of the Quality Assurance Prograa. and to determine the effectiveness of the program."
Contrary to the above, Bechtel audit No. J-19-01 conducted on 6/27/80 was not properly " planned" or " carried out" as shown by the following:
1.
The only adverse finding in the audit cited a drawing for specifying undersize fillet welds because the auditor apparently compared the drawing to the wrong fabrication standard.
2.
The adverse finding was identified in the audit report as "being tracked by QAR F-148" when in fact QAR F-148 had been closed on 5/30/80.
This is an infraction.
B.
As required by Criterion V of 10 CFR 50, Appendix B, and implemented by PSAR Section 17.1.5; " Activities affecting quality shall be prescribed by documented instrue; ions, procedures, or drawings...and shall be accomplished in accordance with these instructions, procedures or drawings. Grand Gulf Specification No. 9645-M-204, Revision 26, " Design Specification For Field Fabrication and Installation of Nuclear Service Piping and Instrumentation For MP&L GGNS", paragraph 5.2.1g, requires temporary field marking to be accomplished with authorized products.
Contrary to the above, on August 19, 1980, "Sanford No. 1501" black markers were being used, by craf t personnel, without authorization, to marh stainless steel safety related components.
This is a deficiency.
C.
As required by Criterion IX of 10 CFR, Appendix B, and implemented by PSAR Section 17.1.9, " Measures shall be established to assure ther special pro-cesses, including... Nondestructive testing are controlled and accomplished

Mississippi Power and Light Company Appendix A Grand Gulf Notice of Violation 06, Revision 6, Sheet 2 of ' limits component nozzle transition surface angle with respect to the piping center line, at the toe of an abutting weld to 30 degrees maximum.
Contrary to the above, on August 20, 1980, the transition surface angle on residual heat removal system valve F-051B nozzle with respect to the piping

center line was 45 degrees at the toe of weld FW-35 on Grand Gulf drawing 9645-M1347A, Revision 21.
