SUPPLEMENTAL INFORMATION FOR LER NP-33-78-57 i

DATE OF EVENT: May 8, 1978 FACILITY: Davis-Besse Unit 1 IDENTIFICATION OF OCCURRENCE: 3reaker contact failure en electric fire pump motor breaker.

Conditions Prior to Occurrence: The unit was in Mode 6, with Power (MWT) = 0, and Load (MWE) = 0.

Description of Occurrence: At 1000 hours0.0116 days <br />0.278 hours <br />0.00165 weeks <br />3.805e-4 months <br /> on May 8, 1978, while performing Surveil-lance Test ST 5016.02, " Fire Protection System Electric Pump Weekly Surveillance Test", an operator attempted to start the fire pu=p.

When the operator pressed the start pushbutten, the starter contacts arced causing a fire inside the starter cabinet. The Fire Brigade was summoned and they put out the s=all fire in the The electric fire pump was declared, inoperable placing the unit in the starter.

Action Statement of Technical Specification 3.7.9.la which requires both the diesel and electric -fire pump at all times. The diesel fire pump was operable and availa-s

)ble. A Maintenance Work Request was prepared to fix the breaker.

Designation of Accarent Caus'e of Occurrence: Component failure of the starter contacts was the cause of this occurrence.

Analysis of Occurrence:

There was no danger to the health and safety of the public or to unit personnel. The loss of the electric fire pump did not affect the opera-tion or availability of the Fire Protection System as the diesel fire pump was available and operable.

Corrective Action

The defective contacts were replaced and the starter cabinet cleaned up.

The electric fire pump was tested and returned to service at-1630 hours on May 10, 1978, removing the unit from the Action Statement of. Technical Specification 3.7.9.1.a Failure Data: There have been no previous si=ilar occurrences.
