Appendix A NOTICE OF VIOLATIOD l

Commonwealth Edison Docket No. 50-237 Company Based on the inspection conducted on April 5, 6, 18, 19, and 24-26, 1978, it appears that certain of your activities were in noncom-pliance with NRC requirements, as noted below. Items 1 and 2 are infractions.

1.

Unit-2 Technical Specifications, Section 6.2.A.6, requires that preventative and corrective maintenance operations which could have an effect on the safety of the facility shall have detailed written procedures which shall be prepared, approved, and adhered to.

Contrary to the above, during a routine outage of Unit-2 during November 1977, the thermocouples for the MSIV pilot valves were removed from the individual MSIV's during per-formance of routine preventative maintenance procedure DMP 233, " Main Steam Isolation Valve Preventative Maintenance,"

without the use of detailed written instructions. The licensee's corrective action to prevent recurrence has been reviewad and appears to be adequate; ther efore, no further response is required.

Lnit-2 Technical Specifications, Section 4.2.A.1, requires that operation of the reactor and other systems and components involving nuclear safety of the facility be performed in accordance with detailed written procedures.

Contrary to the above, on March 21, 1978, the licensee performed a switching operation not in accordsnce with DOP 6900-8, " Station Battery Isolation and Return to Normal Discharge Testing," which caused the loss of 250 VDC supply to the Unit 2 Reactor Building and resulted in the Unit 2 Isolation Condenser being inoperable for approximately one hour. The licensee's corrective acrion to prevent recurrence has been reviewed and appears adequate; therefore, no further response is required.
