APPENDIX A NOTICE OF VIOLATION Northeast Nuclear Energy Company Docket No. 50-423 Millstcne Nuclear Power Station, Unit 3 License No. CPPR-113 As a result of the inspection conducted May 8-29 and June 1-5, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violation was identified:
10 CFR 50, Appendix B, Criterion V states, in part, that:
" Activities affecting quality shall be... accomplished in accordance with (the prescribed) instructions, procedures, or drawings."
The Northeast Utilities QA Program Topical Report, Section 5.2, states, in part, that:
"During construction the contractor programs are re-quired to clearly delineate the actions to be accomplished in the...
control of procedures, instructions and drawings and the methods for complying with 10 CFR 50, Appendix B."
The Stone and Webster Quality Standards QS-10.31, states in section 4.1.2.A that the construction department is responsible for installing the mechanical equipment and components in accordance with the equipment installation specifications, drawings, and manufacturer installation instructions.
Contrary to the above, as of_ May 16, 1981, three mechanical snubbers were installed in the reactor plant component cooling water system without the-use of the required supplier's instruction manual (No.
PHD-7594-1, Rev. 3, issued 10/12/79).
l This is a Severity Level IV Violation-(Supplement II).
Pursuant to the provisions of 10 CFR 2.201, Northeast Nuclear Energy Company is hereby required to submit to this office within thirty days-of tho date' of this Notice, a written statement or explanation in reply, including:
(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved.

shall be submitted under oath or affirmation. Where good cause is shown, I
consideration will be given to extending your-response time.
