50-281 REPORT NO:

81-059/03L-0 EVENT DATE:

08-19-81 LOSS OF POWER TO SMOKE DETECTOR PANEL 1.

DESCRIPTION OF EVENT

On August 19,1981, with the unit at 100% power, the smoke detector system trouble alarm was received.

Initial evaluation determined the alarm to be spurious. Subsequent investigation, several hours later, determined the panel had lost power. Hourly walkdowns were immediately established.

Repair of the system initiated. Failure of the smoke detector panel is contrary to T.S.3.21.A.l.

This event is reported in accordance with T.S 6.6.2.b.(2).

2 PROBABLE CONSEEQUENCES AND STATUS OF REDUNDANT EQUIPMENT:

The fire detection instrumentation ensures that adequate warning capability is available to promptly detect and locate fires in their early stages and thereby reduce the potential for damage to safety related equipment.

Hourly walkdowns of the affected areas were established when trouble was found to be in the smoke detector power supply. All applicable fire suppression systems were operable during the time that the smoke detector system was in-operable.

~

The health and safety of the public were not affected.

3.

CAUSE

The cause of this event was a faulty power supply card in the battery charger.

4.

IMMEDIATE CORRECTIVE ACTION

Hourly walkdowns of the affected areas were established and the faulty power supply card was ordered.

5.

SUBSEQUENT CORRECTIVE ACTION:

The defective power supply card was replaced af ter receipt of the replacement part. The system was returned to service on August 25,1981.

6.

ACTION TAKEN TO PREVENT RECURRENCE:

None required.

7.

GENERIC IMPLICATIONS:

None.
