F APPENDIX A NOTICE OF VIOLATION Philadelphia Electric Company Docket Nos. 50-277, 50-278 Peach Bottom Units 2 and 3 License Nos. DPR-44, DPR-56 During inspection on January 1 - February 2,1982, and in accordance with the Interim Enforcement Policy, 45FR66754 (October 7,1980), the following viola-tions were identified.

A.1 10CFR71.5 prohibits delivery of licensed materials to a carrier for trans-port unless the licensee complies with 49CFR170-189. 49CFR173.24(a) re-quires that a package used in such transfer have its contents limited so that, under nomal transport conditions, the packaging effectiveness will not be substantially reduced.

Contrary to the above, on January 5, 1982, a package containing 0.44 milli-curies of licensed radioactive material was delivered to a carrier for transport and its contents were not sufficiently limited: packaging effec-tiveness was substantially reduced during normal transport when a puncture of about 2-inch diameter was made in the container's side by mat 9 rial with-in the package.

This is a Severity Level III Violation (Supplement V) applicable to DPR-44 and DPR-56.

A.2 10CFR19.ll, " Posting of Notices to Workers," requires posting, 'Athin two working days after receipt, of any notice of violation involving radiolo-gical working conditions.

Contrary to the above, Item A.1 in a Notice of Violation dated January 5, 1982, involving radiological working conditions and received by the 11cen-see on January 8, 1982, was not posted until January 22, 1982.

This is a Severity Level VI Violation (Supplement IV) applicable to DPR-44 and DPR-56.

A.3 Technical Specification 6.11. " Radiation Protection Program," requires ad-herence to radiation protection procedures. Procedure HP0/C0-4, Revision 17, December 18,1981, " Radiation Work Pemits," requires personnel who enter an area covered by a radiation work pemit (RWP) to sign the RWP and enter additional data, including date, social security number, authorized exposure, time of entry, and pocket dosimeter reading.
