Desclintion of Occurrence:

Investigation, initiated uhen movment of the HPCI steam i

supply line was noted, revealed that the steam supply line drain pot was not draining through the out1ct steam trap nor had the drain pot i

high 1cyc1 switch, LS-3-23-90, actuated to open the steam trap bypass valve. This allowed accumulation of water in the steam supply line.

i Designation of Annarent Ccuse of Occurrence:

l Failure of the stecn trcp to drain and failure of the drain

{

pot level switch to trip on hi h level.

Annlysis of Occurrence:

At the time the accumulation of unter was discovered, the

,)

reactor was operating at steady state conditions with no operational or surveillance requircnents to mn the HPCI systm. In the unlikely event that the HPCI was initiated and it had beci noccssary to shut it down, the RCIC and ADS systms were availabic for high pressure conditions.

Therefore the safety ir:plicatiens of this occurrence are minimal.

1 Corrective Action:

The stern trop was dismentled and it iras fcund that the trap orifice uss plugged with a small cmount of dirt.. The trap was cleaned, 1

reassmbled, returned to sorrice and is operating prcperly. The IcVel switch adjustacnt linkage uns found to be binding.

The switch mechanisn was realigned, returned to sertice, and is operating i

satisfactorily.

I Failure Data:

j None.
