Failure of the 11PCI 5tean Supply Line Drain System to properly drain.

Octaditions failure to Occurrence:

Unic 3 at approximately 100* power.

Description of Occurrence:

Investigation, initiated when covecent of the HPCI steam nupply line van noted, revealed that the stecn supply line drain pot was not draining through the outlet stean, trop nor had the drain pot high Iceci switch, LS-3-23-90, actuated to open the e. team crap bypass valve, allowing accumulation of water in the steam supply

line, Designation of Apparent Cause of Occurrence:

Failero of the stean trap to drain and failure of the drain pot level switch to trip on high Icvel.

Analysis of Occurrence:

At the tice the accu:.ulation of water was diccovered, the reactor was operating at stendy a: ate conditione with no operational or surveillance requirements to run tbs UNI systen.

In tho event the UFCI was autorutically actusted cnd it was necesr.ary to nhut the UPCI down, the RCIC and ADS syrtens were available for high pressure conditions. Safety icplications cro minical.
