Dear Mr. Martin:

Following the NRC Region !Y Special Team Inspection, Report 50-407/88-1 and 50-07J3/88-01, the University of Utah TRIGA Nuclear Reactor, License R-126, Docket No. 50-407, was ordered shut down pending resolution of various inspection items.

The Licensee, :s a result of this action, perfor' sed the following:

Responded to Notice of Violation and Deviation at an Enforcement Conference on 28 March 1988, to Robert D. Martin on 8 April and 14 June 1988, and to James M. Taylor on 4 August and 25 August 1988.

Secured the services of an additional senior staff person to serve as Reactor Supervisor (Dr. K. C. Crawford).

The Reactor Safety Comittee (RSC) has reviewed the Inspection Report and required modifications, new procedures, additional equipment installation, etc. to resolve the findings of the Inspection.

The Licensee relieves the facility now meets all NRC operational requirements under the license and, ith the arrival of K. C. Crawford on 3 October 1988, requests that the snut-down order be lifted and the facil-ity be allowed to resume operations on 3 October 1988. The RSC will also review all previous actions before resumption of operations and when satisfied that the facility is in full compliance with the requirements of its operating license will authorize the staff to operate the TRIGA reactor.
