1, 02/27/97 7.2 IMMEDIATE NOTIFICATION Information which will be required by off-site agencies during any emergency is noted in Section 7.1, on NRC Form 361A, " FUEL CYCLE & MATERIALS EVENT NOTIFICATION WORKSHEET",

and on NRC Region II Form,

" PROTECTIVE MEASURES STATUS".

SC-DHEC The South Carolina Department of Health and Environmental Control, Nuclear Emergency Planning Section, will be notified within fifteen (15) minutes of incident classification under the following conditions:

- classification of an event as an ALERT or SITE AREA emergency;

- incidents where off-site medical treatment is necessary, whether due to occupational or general public exposure to radiation;

- any discharge of materials to the environment which are above applicable limits and may lead to public relations problems;

- incidents which are reportable to the US-NRC including

(

abnormal security occurrences;

- any incident where off-site effects may cause exposure of the public to radiation or radioactive materials.

RICHLAND COUNTY The Richland County Department of Emergency Services will be notified immediately after SC-DHEC notification of an event classified as an ALERT or SITE AREA emergency.

US-NRC The United States Nuclear Regulatory Commission will be notified immediately after the state and county are notified, and in any case within one (1) hour of the classification of an event as an ALERT or SITE AREA emergency.

- Lost, stolen or missing material equal to or greater than 1000 times the quantity specified in Appendix C of 10CFR20 under such circumstances that an exposure could result to persons in unrestricted areas.

Exposure of any individual equal to or exceeding a total effective dose equivalent (TEDE) of 0.25Sv (25 REM),

a shallow dose equivalent to the skin or extremities of 2.5Gy (250 RAD), or an eye dose equivalent of 0.75Sv (75 REM).

The release of radioactive material inside or outside of a restricted area so that had an individual been present for 24

hours, the individual could have received an intake five times the annual limit of intake (ALI).

(This does not apply to locations where personnel are not normally stationed during routine operations such as process enclosures).

When discovering an event which prevents implementing immediate protective actions necessary to avoid exposure to radiation or radioactive material that exceeds regulatory limits, or releases which exceed regulatory limits.

- Any incident where off-site releases may cause exposure of the public to radiation or radioactive materials.

- Any incident requiring off-site treatment of either employees or the general public.

- Security intrusions to the site.

RICHLAND MEMORIAL HOSPITAL The Radiation Emergency Response Staff of the hospital will be notified immediately of any serious injury or exposure to any individual from radiation or contamination by radioactive material whether accident or emergency event related.

i RICHLAND COUNTY SHERIFF The Richland County Sheriff's office will be notified immediately if assistance is required in controlling access to or egress from an area involved in an emergency.

This may include assistance in evacuating members of the general public.

WESTINGHOUSE Corporate Energy Systems Emergency Committee, CNFD General l

Manager and Corporate Crisis Center will be notified of any event classified as an ALERT or SITE AREA emergency.

regarding all environmental releases or spills. **

1, 02/27/97 AMERICAN NUCLEAR INSURERS American Nuclear Insurers Mutual Atomic Energy Liability Underwriters (ANI-MAELU) will be notified immediately of any emergency at the ALERT or SITE AREA classification level, and for any condition which requires the assistance of any off-site emergency response group providing care for the injured or protective actions for the general public.

7.3 EIGHT HOUR NOTIFICATION DEPARTMENT OF LABOR The South Carolina Department of Labor must be notified within eight hours of an emergency or accident which results in a fatality, or hospitalization of three or more persons.

This eight hour notification also applies if the hospitalization or death occurs within a

thirty (30) day period after occurrence of the emergency / accident.

The following information will be required:

- number of individuals involved;

- number of fatalities;

- number of persons hospitalized;

- names of persons involved;

- time incident occurred;

- individuals / organizations notified;

- family (ies) notified;

- situation status.

7.4 TWENTY-FOUR HOUR NOTIFICATION US-NRC The US-NRC will be notified within twenty-four hours of any incident resulting in the following:

- exposure to any individual equal to or exceeding a TEDE of 0.05Sv (5 REM), a shallow dose equivalent to the skin or extremities exceeding 0.5Sv (50 REM),

or an eye dose l

equivalent of 0.15Sv (15 REM);

- unplanned contamination event which 1) restricts access to contaminated areas for more than 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />, 2) involves a quantity of material five times the lowest ALI, or,

3) restricts access for reasons other than decay; an event in which safety equipment fails to function according to regulation or license requirements;

- an event in which unplanned medical treatment beyond first aid is required for an individual with fixed or removable contamination on his body exceeding 500dpm/100cm2;

- an unplanned fire or explosion damaging licensed material or any device or container or equipment containing licensed material;

- an incident which may not result in radiation exposure, but which creates serious public relations problems.

7.5 THIRTY DAY NOTIFICATION US-NRC A written report must be submitted to the US-NRC within thirty (30) days of occurrence for the following:

l

- any incident requiring immediate or 24-hour notification;

- each exposure of an individual to radiation or concentrations in excess of any applicable limit in 10CFR20 or in the US-NRC facility license conditions; levels of radiation or concentration or radioactive material (not involving excessive exposure of an individual) in an unrestricted area in excess of 10 times any applicable limit noted in 10CFR20, or in the conditions specified in the US-l NRC facility license;

- a telephone call to the US-NRC Emergency Operations Center reporti.ig any lost, stolen or missing licensed material in a amount greater than 10 times the quantity specified in Appendix C of 10CFR20.
