In order to complete our review, we need the
following additional information:
You should state the means of determining when a trainee has satisfactorily
completed your in-house training program and is capable of carrying out the
radiation safety responsibilities required by the licensee. Normally, this
is accomplished by means of a written exam.
If so, you should submit:
a sample examination

an answer key
l
the required score for passing the exam
l
the preinstruction (or re-examination) to be given in
areas in which individuals are deficient
We will continue the review of your amendment request upon receipt of this

In order to continue prompt review of your application, we
request that you submit your response to this letter within 30 calendar days
