Houston Lighting.&-Power Company Docket:
50-499 iSouthTexasProject(STP), Unit 2-Operating License: NPF-78 During an NRC inspection conducted January 1-31, 1989, a violation of NRC i
requirements was identified. The violation involved the failure to accomplish i
an activity affecting quality in accordance with documented procedures.
In-
- accordance with the " General Statement of Policy (and Procedure for NRC1989),thevio Enforcement Actions," 10 CFR Part 2, Appendix C
- below:
,10 CFR Part 50, Appendix B, Criterion:Y, requires that activities affecting quality shall be prescribed by documented instructions, procedures, or drawings of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.

requires, in part, that alignment changes or_ alterations in_ status of components / systems identified as safety-related be independently verified-when a' component / system is returned to service following_a period when the component / system was inoperable due to maintenance or testing or when a component / system is removed from service or altered from its normal configuration, n Contrary to the' above, Station Procedure OTEP03-FP-0006, Revision 0,
" Relay Room Halon Concentration Test," did not include provisions for independent' verification that the control room envelope makeup fan air inlets had been.estored to normal configuration after testing. This r
oversight ' led to the plastic sheeting being left on the Train C control room envelope makeup fan inlet thereby rendering that train inoperable.
This is a Severity Level IV violation.
