This lette r ref ers to two investig ations co nduct ed by t he U.S. Nuclea r Regula tory Co mmission (NRC) Office of Investig ations ( OI) at the Holt ec Decom missioning Inter nation al, LLC (HDI)
Oyster Cre ek Nuclea r Gene rating S tation (Oyster C reek). T h e investig ations, whic h w ere complete d on S ept embe r 23, 202 1, and Octob er 14, 202 1, res pectively, inv olved sec urity matters that are co nsider ed sensitiv e infor mation. Th eref ore, details of the inv estigatio ns are not include d in this le tter but a re desc ribed i n the non-public encl osure.
B ased on th e res ults of th e NRC OI investi gations, the NRC staf f dete rmine d that multiple examples of a S eve rity Level IV viol ation of NRC requi reme nts occu rred, but t hat th e occurre nces did not r esult fr om th e willful actions of any of th e involved individuals. Th e NRC is treating this violatio n as a non-cit ed violatio n (NCV ) consist ent with S ecti on 2.3.2.a of the E nforceme nt P olicy. T he det ails of th e violation, includin g the assessme nt of its si gnificance, are pr ovided i n the enclosur e. The NRC not es that HDI took i mmedia te cor rective actions t o address any secu rity-r elated v ulner ability.
