1 l OVESTIONS FOR INTERVIEW j OF TAK ! POST-INTRODUCTION RELATED For purposes of this interview, the fire barrier penetration seal product l being discussed during this interview will only be the Dow Corning 3-6548 ' Silicone RTV Foam and we'll refer to it as just silicone foam. When was the silicone foam made into a two part product? Did a formula modification change occur to make the silicone foam a two part product? FORMULA MODIFICATION CHAf4GE RELATED l How many times since you've been employed with Dow Corning (1975) has the 3-6548 Silicone RTV Foam formula changed or been modified? When did the (these) formula modification change (s) occur? ' What specifically did the formulation modification change include? Was their ' a specific chemical compound that changed? Once TAK has read over the Dow Corning formula modification letter of November 12, 1994, ask the following questions: What is the report no. of the e.-814 report referenced in the November 12, 1984 letter? When was the test conducted? By who and at what lab? What was the purpose of conducting the test? Who authorized the test? What were the results of the testi % would like copies of the test? Any other specifics obtain. Were all configurations that failed subsequently retested? Did any of the new formula and/or old formula silicone foam material not meet the E-Il9 conditions of acceptance test criteria? Were these test reportr provided to customers to demonstrate the adequacy of the silicone foam material as a fire barrier penetration seal with a 1 hour or 3 hour fire resistance rating? If known, are any of the Dow Corning tests being used by nuclear utilities? Do you know if any nuclear utilities were notified of the Dow Corning formula modification change? Are you aware of any problems with the silicone foam that were not existing prior to the formula modification change but that began to occur subsequently? Are you aware of fire barrier penetration seal tests conducted on the same seal system configuration that passed the fire test prior to the formula modification change but failed the E-Il9 conditions of acceptance test criteria after the formula modification change? awa

i i i i TRANSCO FIRE TEST RELATED Questions related to the Millstone tests and observations thereof. ) i SEAL DEFICIENCIES RELATED In 1988, the NRC issued an Information Notice numbered 88-56 that identified problems encountered with silicone foam fire barrier penetration seals installed by the Promatec Company at Wolf Creek Nuclear Plant suchas voids, I shrinkage, and lack of fili. Based on your knowledge and experience, have you ! encountered these types of problems? Based on your experience, what did you believe the problems with voids, shrinkage & lack of fill were primarily a result of (i.e. manufacturer related ; versus poor installation technique and/or poor'Q.C.)? i 9" SEAL RELATED Are you aware of a 9" fire barrier penetration seal system that does not leave in place the damming board, which has successfully passed the ASTM E-119 conditions of acceptance test criteria for a three hour fire rating? If so, please identify the-test report (s) number and date of the report or test date. Do you recall if BISCO has on file any fire barrier penetration seal fire test l that demonstrate a 9" penetration fire seal successfully passed the E-119 conditions of acceptance test criteria for a three hour fire rating? Lastly, are you aware of any fire barrier penetration fire seal system at a nuclear facility that is currently not qualified by passing the E-119 conditions of acceptance test criteria?

+ , INTERVIEW OF CLAYTON W. BROWN Introduction 9? Today's date is September 20, 1994, at approximately 10+00 a.m. For the l l record, this is an interview of CLAYTON W. BROWN, spelled B-R-0-W-N, who i
- is currently a technical consultant to Brand Fire Protection Services.
i The location of this interview is the Executive Conference Room of the U.S. j ! Nuclear Regulatory Commission's Region III Headquarters Office, located at 801 Warrenville Road, Suite 255, in Lisle, Illinois. Present at this interview ! are Richard C. Paul, spelled P-A-U-L, Senior Investigator, and Joseph M. Ulie, spelled U-L-I-E, Investigator, both with the U.S. Nuclear Regulatory 1 Commission, Office of Investigations. As agreed, this interview is being tape i recorded by Court Reporter Ronald N. LaGrande. The subject matter of this interview regards a fire barrier material known as Dow Corning numbered 3-6548 Silicone RTV Foam, which is used in the nuclear industry, in part, as a fire i resistive barrier to sehl penetration openings so as to separate fire areas. This interview will also allow Mr. Brown time to provide any addl. information pertinent to the subject matter of the interview, for which Mr. Brown may wish to make a part of this record and proceeding. Mr. Brown, please stand and raise your right hand. "Do you swear that the information that you are about to give is the truth, the whole truth, and nothing but the truth, so help you God?" Please be seated. 70 US

changes Bisco and its successor, Brand, went thru, and your position when these changes occurred? i
~ For this interview, the names Brand and Bisco will be used interchangeably.
Does the fire barrier penetration seal product used by Bisco at nuclear plants have the specific nan,a & no, designation of Dow Corning 3-6548 Silicone RTV i Foam? For purposes of this interview, the Dow Corning 3-6548 Silicone RTV ' Foam will be referred to as just silicone foam.
- How many years ha' v e you been involved in the penetration fire barrier seal i
business?i7 And how many of those years have you been specifically involved in 1 installing penetration seals at nuclear power plants?g
- Approximately, how many nuclear pnwer plant units did Bisco have a contract to install penetration seals?ff (.

- Did Bisco co-oevelop the silicone foam material with Dow Corning? Please provide the background details in summary form regarding the development of the silicone foam.
Formula Modification Chanae Related
- Please read over this November 12, 1984 letter to its Power Applicators, which identifies a silicone foam formulation change. Do you recall receiving a copy of this letter? -If not, are you aware of any formula modification change that occurred to the silicone foam product about 1984 or 1985 timeframe? -'The letter specifies fire endurance testing was conducted that showed the modified formulation met ur exceeded the properties of the existing silicone ew of these tests concur with Dow Cornings statement?

- Rich, do you have any further questions within this area?
Gao and Void Problems
- Are you familiar with seal deficiencies suchas cracking, spliting or gaps, lack of fill, & voids being identified with as installed seals, which was the subject of an NRC IEN No. 88-567 ' Based on your experience, wha do ou elieve these problems (with voids, shrinkage & lack of fill) wer a result o 1. .
