FORMULA MODIFICATION CHAf4GE RELATED l How many times since you've been employed with Dow Corning (1975) has the 3-6548 Silicone RTV Foam formula changed or been modified? When did the (these) formula modification change (s) occur? ' What specifically did the formulation modification change include? Was their ' a specific chemical compound that changed? Once TAK has read over the Dow Corning formula modification letter of November 12, 1994, ask the following questions: What is the report no. of the e.-814 report referenced in the November 12, 1984 letter? When was the test conducted? By who and at what lab? What was the purpose of conducting the test? Who authorized the test? What were the results of the testi % would like copies of the test? Based on your knowledge and experience, have you ! encountered these types of problems?
