A. FacilityModifications: None B. Results ofTests and Inspections:

Tests concerning the reactor structure (Section 4.4) were continued as recommended by the Reactor Safety Committee. These inspections revealed no water leakage, no excessive corrosion, and no unusual radiation. Annual Checks were started in February. The tests were terminated beca.ase of a power channel failure. Replacement parts for the power channel electrical circuit were ordered, but not received by the end of the reporting pedod.The power channels have'been npaired and annual tests will be completed when the contml system is returned to normaloperation.

C. CorrectiveMaintenance:

The defective power channel has been repaind and a rod drop measurement circuit is being designed and installed to measure ore drop times as required under the annual maintenance tests. Maintenance testing is incomplete at the end of the 1 present repuding period.

Enclosed is a newly revised and approved Charter for the Reactor Safety Committee at the University of Utah Nuclear Engineering Laboratory. This Chaner governs the activities of the Reactor Safety Committee which oversees the operations of the TRIGA Reactor ( Docket No.

50-47, License No. R-126) and the AGN Reactor (Docket No. 50-72, License No. R-25).

ReactorSafetyCommittee Charter I. Membership and Organization The Reactor Safety Committee is one of the University's regularly constituted standing committees. Appointments to the Committee are made through the office of the Academic Vice President. The Committee reports to the Vice President for Research.

The Committee shall be composed of at least five members knowledgeable in fields that relate to nuclear reactor safety. The Committee shall consist of the Reactor Administrator, who shall be the Chairman, the Reactor Supervisor, the Radiation Safety Officer and at least two other 4

members not directly involved in reactor operations but knowledgeable in fields related to nuclear reactor safety.

One member of the Committee other than the Chairman will be appointed by the Chairman to serve as Secretary of the Committee.

All significant deliberations and all actions of the Committee shall be reported in the minutes by the Secretary in sufficient detail for an independent reviewer to understand the nature and consequences of Committee actions. The Secretary will assure that minutes of each Committee meeting are made and copics distributed to all members after the meeting.

II. Duties and Responsibilities

1. Review and appmve safety aspects associated with the use of the reactor facility; in conformance with NRC and University regulations;
2. Review and appmvc all proposed experiments and procedures as required, and changes thereto, and modifications to the reactor and its associated components;
3. Determine whether proposed experiments, procedures or modifications involve umeviewed safety questions, as defined in 10 CRF 50, Part 50.59 (c), and are in accordance with the written Technical Specifications;
4. Conduct periodic audits of pmcedures, reactor operations and maintenance, equipment performance, and records;
5. Review all reported abnormal occurences and violations of the Technical Specifications;
evaluate the causes of such events and the corrective action taken, and recommend measures to pievent recurrence;
6. Reports of findings and recommedations concerning the above stated items 1-5, whether performed by indivdual Committee members, or by appointed subcommittees, are to be made to the Reactor Administrator.

Ill. Meeting

'Ihe Committee shall meet on call of the Chairman and shall meet at least once in each calendar quarter.

IV. Quorum A quorum for conducting the business of the committee shall consist of a simple majority ofits members; provided that this majority includes the Reactor Administrator, the Reactor Supervisor and the Radiation Safety Officer, or their designates.

V. Parliamentary Rules Robert's Rules of Order shall be followed in conducting all meetings of the Committee.

VI. Order of Business The official agenda, together with copies of all proposals for significant Committee actions (e.g., new experiments, proposed amendments to licenses or technical specifications, etc.), as distributed prior to the meeting, shall constitute the principal business at each meeting.

(1) Rollcall; (2) Action on the minutes of the preceding meeting; (3) Report by the Reactor Administrator; (4) Report by the Reactor Supervisor, (5) Report by the Radiation Safety Officer, (6) Reports by otherindividuals or subcommittees; (7) Appmval of experiments, procedures etc.;

(8) Review of communications, audit of mcords and reactor operations, logs and miscellaneous matters.

VII. Records Documentation of the activities of the Committee shall be maintained. The documentation shall include the names and qualifications of the members, the agenda and approved minutes of all Committee meetings, Committee actions, and copies of all cormspondence and reports to or from the Committee. One complete file should be ycyared and transmitted annually to the University Archives. The original file shall be maintained by the Chairman.
