CASE STUDY REPORT - A REVIEW 0F.M0 TOR-0PERATED val.VE PERFORMANCE The Office for Analysis and Evaluation of Operational Data '(AE0D) has finalized its case study on a review of motor-operated valve performance. .The final report addresses the peer review comments provided by NRR, IE, RES, the Regions, INP0, NSAC, M0 VATS Inc., and Toledo Edison Company. The report brings together previous NRC studies and other related documents (AE0D reports and IE bulletins, circulars, and information notices), reviews approximately 1200 valve operator events from 1981 to mid-1985, and incorporates new data from an NRC valve testing program that utilized' signature tracing techniques on valve assemblies in operating plants. This report also serves as the AE0D response to staff action item 6(1) of the August 5, 1985 ED0 memorandum: " Staff.

Actions Resulting from the investigation of the Davis-Besse Event (NUREG-1154)".

Our case study report concludes that current methods and procedures-at many operating plants are not adequate to assure that valves will operate when .

  1. ~ Multiple Addr:ss u s existing (plant proceduras intended to assure operability, such as surveillance testing) testing or plant technical plant operatorspecifications observations.and ASME Thus, Code, Section assurance of valve XI inservice operability appears to be strongly dependent upon diagnostic capability to assess and evaluate failures to operate so that root causes of failure--including switch setpoints--are correctly determined and proper changes implemented. Further, the issue of valve assembly performance and reliability is a complex subject that involves several technical disciplines. The report makes four recommendations.

In view of the common NRC and industry objective of resolving identified safety issues relating to operation of nuclear plants in the most effective manner, we understand that the ED0 will request that NUMARC undertake appropriate industry wide initiatives to address the findings, conclusions and recommendations in the AE0D case study. The report is being transmitted to appropriate NRC offices for information and possible future NRC action if an effective industry improvement program is not undertaken and implemented in a timely manner.
