SUBJECT:

CASE STUDY REPORT - A REVIEW OF MOTOR-0PERATED VALVE PERFORMANCE The enclosed case study report is our response to staff Action Item 6(1) that was transmitted with the August 5, 1985 ED0 memorandam: " Staff Actions Resulting from the Investigation of the Davis-Besse Event (NUREG-1154)." This action item requested that AE0D conduct a review of failures of safety-related motor-operated valves and provide an assessment of pertinent failure modes affecting valve performance under design basis conditions.

We have completed the study requested by this action item. Approximately 1200 events from the Sequence Coding and Search System and the Nuclear Plant Reliability Data System were identified and evaluated in the study. The time period considered was January 1981 to mid 1985. The pertinent failure modes have been identified by~our review and assessment.

The enclosed final report documents our findings, conclusions, and recommendations resulting from the study. This report reflects comments received as a result of the peer review process. We understand that you will transmit the report to NUMARC with a request to undertake appropriate industry initiatives to address the motor-operated valve assembly performance related problems and recommendations identified in the report. By separate memorandum, the enclosed AE00 report has been forwarded to NRR, IE, RES, and Regional Administrators for information.
