Technical Specification Violation at The Ohio State University Research Reactor Docket No. 50-150, License R-75, Resulting in a Reportable Occurrence to the NRC
Dear Sir:
Technical Specification 3.5(1) concerning the Reactor Building ventilation system states that an exhaust fan with a capacity of at least 1000 cfm shall be on whenever the reactor is operating. This'is one of several limiting conditions for operation listed in section 3.0 of the Technical Specifications. Contrary to this, at about 1300 hours0.015 days <br />0.361 hours <br />0.00215 weeks <br />4.9465e-4 months <br /> on February 24, 1989 the exhaust fan was not operating when the reactor was critical at 10W. Operating in violation of a limiting condition is a reportable occurrence and requires the following:
- 1. Reports to NRC
- a. Telephone or telegraph to NRC Region III ASAP and no later than the next working day (this was completed at 1540 on February 24, 1989 by telephone communication to Donald E.
Miller).
- b. Written report to NRo in Washington with a copy to Region III within 14 days (this etter represents the written report).
- 2. Actions
- a. Reactor conditions shall be returned to normal, or the reactor shut down, to correct the occurrence (the Reactor was shut down by a manual scram as soon as the operator was made aware of the situation at about 1304).
- b. The Director of the Nuclear Reactor Laboratory (NRL) shall be notified ASAP and corrective action shall be taken before resuming the operation involved (Don W. Miller was notified at about 1400 on February 24, 1989 and the switch controlling the fan was returned to the "on" position).

- c. A written report to the Director of the NRL and the Reactor Operations Committee (ROC) for review and approval including:
- 1. Cause: Apparently the switch was snagged by a belt loop as an individual left the control room at about 1300. __
This was noted by an SRO in Room 104 who heard a relay outside his office trip. He brought this to the attention of another SR0 who realized the vent fan was turned off. This information was given to the console operator who immediately scrammed the reactor at 1304,
- 2. Corrective action: The reactor was scrammed and the fan switch was returned to the "on" position.
- 3. Actions to preclude or reduce the probability of recurrence: A guard shall be attached over the switch to prevent or reduce the probability of this occurring again. It shall be installed on or before March 6, 1989.
The required written report was submitted to Don W. Miller, Director of the NRL, on February 27, 1989. It was reviewed and approved by the Reactor Operations Committee at a meeting held on March 2, 1989. We believe these actions meet the requirements of the OSURR Technical Specifications and that the reacto'r can continue to be operated in a safe manner.
