SUBJECT: NRC INTEGRATED INSPECTION REPORT 50-327/98-06 AND 50-328/98-06
AND NOTICE OF VIOLATION

Dear Mr. Scalice:

This refers to the inspection conducted on April 26 through June 6. 1998, at
the Sequoyah Nuclear Plant facility. The enclosed report presents the results
l
of this inspection.
.During the inspection period, your conduct of activities at the Sequoyah {
facility-was generally characterized by good operations. However, negative
findings were identified for a failure to locally monitor the turbine driven -
auxiliary feedwater.pum) during a maintenance run and for tapping on a valve
that had failed to stroce. The conduct of maintenance activities was
considered to be good. However, a weakness was identified for a less than
l effective program for identification of water intrusion into electrical i components. Engineering support was considered to be satisfactory. However,
three violations were identified and are discussed briefly in the following
paragraph and a weakness was identified for a water hammer event- that damaged
'
a Unit 1 steam dump support. Plant support was considered to be good.
However, a violation was identified in the security area and is discussed in
,
the following paragraph.
Based on the results of this inspection, the NRC has determined that
violations of NRC requirements occurred. The violations are cited in the
enclosed Notice of Violation (NOV) and the circumstances sur. rounding them are
described in detail in the subject inspection report. The violations involved
failure to perform ASME Section XI stroke time testing of motor operated
valves, failures request relief form ASME code requirements in a timely
manner.' failure to Jerform an adequate safety analysis prior to implementation ,
of a plant design clange of the oxygen and hydrogen waste gas monitors, and 4
failure to properly control personnel security access at a sally port on four [
different occasions. You are recuired to respond to this letter and should ,
follow the instructions specifiec in-the enclosed Notice when preparing your '
response. The NRC will use your response. in part, to determine whether.
