Contrary to the' above, Station Procedure OTEP03-FP-0006, Revision 0, ,
" Relay Room Halon Concentration Test," did not include provisions for independent' verification that the control room envelope makeup fan air inlets had beenr.estored to normal configuration after testing. This oversight ' led to the plastic sheeting being left on the Train C control room envelope makeup fan inlet thereby rendering that train inoperable.
