Attached is a copy of the Federal Emergency Management Agency (FEMA) report for

      'the partial participation emergency preparedness exercise conducted at the Waterford 3 Steam Electric Station on October 15, 1986.

There were no deficiencies identified during this exercise and only one area requiring corrective action. The State reviewed the draft report and has included the corrective action schedule in this final report. Therefore, the offsite radiological emergency plans and preparedness are adequate to protect f the health and safety of the public in the event of an emergency at the } Waterford 3 Steam Electric Station. In addition, the 44 CFR 350 approval l granted the State on July 15, 1984, will remain in effect. It is requested that you coordinate with the State and Parish agencies to ensure that the area requiring corrective action is corrected in a timely manner.     .7 On December 7, 1979, the President directed the Federal Emergency Management Agency (FEMA) to assume the lead role responsibility for 'all off-site i

nuclear power facility planning and response. , {L. FEMA's immediate basic responsibilities in Fixed Nuclear Facility Radiological

  • Taking the lead in o'ff-site emergency response planning and in the
   - ??                                   review and evaluation of State and local government emergency h                                    plans, and ensuring that the plans meet the Federal criteria set forth in NUREG-0654 FEMA REP-1, Rev.1 (November 1980).

  • Determining whether the State and local emergency response plans '

can be implemented on the basis of observation and evaluation of an

      %                                    exercise conducted by the appropriate emergency response jurisdictions.

  • Coordination of the activities of volunteer organizations and other

The State of f,' Louisiana and three local jurisdictions partially participated in this exercise. The initial O' qualifying Radiological Emergency Preparedness (REP) exercise was conducted February '

                - 8,1984 and all jurisdictions participated on a full-participation basis. The Federal O.             Emergency Management Agency (FEMA) evaluated the off-site radiological emergency                              ,
  ,,,            response capabilities in all three of these exercises.

mg On October 16, 1986, three meetings were held - a 9:00 a.m. meeting of the 11-member Federal evaluation team'to present their preliminary findings to the Acting RAC (". . Chairman, a 1:00 p.m. critique for the State, local and utility officials to present a short overview of the preliminary results of the exercise, and a 5:00 p.m. Public Critique at the St. Charles Parish Courthouse, Hahnville, Loulslana.

State of Louisiana Operations .q r The Loulslana State Office of Emergency Preparedness (LOEP) and the Loulslana

  ';      Nuclear Energy Division (LNED), both located for the exercise at the LOEP Emergency             _
   -      Operating Center (EOC), in Baton Rouge, Loulslana, participated in the Waterford 3 I-i      exercise. Both agencies demonstrated an adequate level of readiness for dealing with a radiological emergency. Due to the limited nature of the partial exercise, only the                      ,

staffing required to demonstrate the objectives designated for the exercise was present - at the EOC. Operations were well managed by the LOEP and LNED personnel present, and the entire (limited) staff was effectively involved in decision making. , Physical aspects of the EOC are adequate to support cesivaous operations and message boards, maps and other displays were posted and effectively used to facilitate

  • 2 the State's response function. Communications facilities, equipment and procedures, in general, functioned in an appropriate manner throughout the exercise. 1

Local governments participating in the exercise were the two Parish j governments located within the 10-mile EFZ, St. Charles Parish and St. John the Baptist ,, Parish, and, to a very limited extent, Jefferson Parish in a support role. These local , participants very capably demonstrated the resources, knowledge and initiative necessary

      -   to alert the populace and to implement protective actions which would protect the                  [j residents in the event of an incident at Waterford 3.

Facilities at the Parish EOCs are, for the most part, adequate and contained ff

 .        space, equipment and other resources to support continuous operations. The St. Charles Parish EOC is equipped with a kitchen and bunking facilities for on-site accommodation and off-shift staff.

Effectiveness of communications equipment, procedures, and staff was established through the activities generated by the exercise and by extensive contacts g with the EOF and other response organizations. a Detailed evaluations of performance at each locality are provided in Section 2 of

    %.            agencies. hilowing the earthquake, at 0820, the Control Room will receive a report that the discharge piping on the "A" LPSI pump is leaking and water was sprayed on the motor. A piping flange has

{ t, cracked and the contents of the line have leaked on to the pump motor. An in-line check valve prevents further leakage. In order for the "A" LPSI pump to be operable, the weld must be repaired. The "A" LPSI pump is declared out of service. Both trains of shutdown cooling c x. are now inoperable. This condition results in the declaration of an Alert at about 0830, i-The Alert declaration initiates the mobilization of the On-site

               emergency Organization (TSC-OSC) and Emergency News Center staff.

The TSC and the OSC staff begin arriving at their respective facilities [ prior to 0900. The earthquake also causes a misalignment of the limit _

  ,                              The communications room staff handled their duties in an effective and

(' professional manner. Communications equipment was adequate, and pz: mary and ' secondary units functioned appropriately, with only minor exceptions, which will be noted. Message handling was done well, and message forms were generally used j'j" effectively and according to the plan. The " bug" in the hotline which was present in the prior Waterford exercise was corrected and the hotline worked consistently. Minor flaws 7, with the hotline involved the variability of volume levels and the difficulty of operating L_ the telephone receiver / sender. The volume level on the hotline telephone and table-top speaker is not consistent and was occasionally weak. To send or receive with adequate

          .           telephone speaker volume, a button on the telephone receiver must be depressed.

y"- Keeping the button depressed can be difficult with long and/or repeated messages. It is desirable to improve the controlability and dependability or volume of the hotline. The ,. copier which is used by the EOC message runner was out of order in the morning,

O Alerting and Notification through EBS messages sent to WCKW by St. John's Parish was accomplished, and copies of the messages were sent to the State EOC. The 'n , 3 Parishes assumed full responsibility for EBS messages. Consistent with the plan, only @ WCKW-FM and her AM sister station issued EBS messages, although the capability to q issue the EBS messages over several other radio stations exists at the State EOC. .; J: . The Assistant Secretary of the LOEP was in charge of the State _ EOC ]f operations. The EOC was also staffed by the Assistant Secretary, Office of Air Quality r and Nuclear Energy, an LNED technical liaison representative, a technical liaison "'4 . representative from the utility and other staff membe.rs of the LOEP. As stated above, y i FEMA objective _ #3 was adequately met. Direction of operations was well managed, ,] 7 including effectively keeping all EOC staff members involved in the decision making process. There was good coordination between the State LNED administrator, located at ", the EOF, and the key staff members at the State EOC. Even though it was not an objective for the exercise, status boards and other displays were effectively used to facilitate the State's response function. The State EOC staff operated out of the

received by the Communications Center at the State EOC, the communications officer wrote on his log to contact FEMA Region VI. He simulated that a call was made. After the controller realized that a call had not been officially made, the communications " f. officer was requested to do so. A call was then made to FEMA Region VI but the verbal request was not precise enough to cause him to give a proper message to FEMA Region , VI. The LOEP officer in charge appropriately then made another call to Region VI i A requesting Federal assistance. Region VI deployed a Deputy Senior official (simulated) and a public information officer (PIO) to the LP&L emergency news center. The Region VI PIO participated in the exercise at the LP&L emergency news center.  ; y \ In summary, the limited objectives of this partial participation exercise were

The utility's Dose - Assessment office is adjacent to the room allocated to the LNED Dose Assessment 3 office, and there is a connecting door. Furthermore, this pair of offices is directly 1 across a corridor from the Radiological Asaressment Coordinator in the Commind Room, and is additionally connected by a dedicated intercom. [ A LNED Liaison Team was dispatched from Baton Rouge to the' EOF when the 9 Site Area Emergency was declared (at 9:52 a.m.) and arrived at the EOF approximately - [ one hour later. At 11:20 a.m., shortly after the declaration of General Emergency, the EOF announced to all parties concerned that they had assumed responsibility for a emergency operations. Due to the limited nature of the partial exercise, with field i

   ;;                       monitoring not being an objective, the LNED team 'had only utility input for dose                                          ,

(, - assessment activity. The utility field team coordinator did, on many occasions, come to i .' the LNED work area and announce a dose assessment (and assume reasonable agreement) ]- before passing the information to the Radiological Assessment Coordinator. The Radiological Assessment Coordinator evaluated the data provided and posted it on the .j. . . i.( status boards near his station in the Command Room. These status boards are easily - - 'T" visible' to the LNED Liaison Officer at the front center of the Command Room. The evaluations made by the LNED - lf Liaison Officer demonstrated the ability to determine appropriate protective measures based on PAGs, available shelter, evacuation time estimates and other appropriate factors. Overall, based on the information given LNED, their response actions were ~k appropriate and conservative. Communications equipment functioned well throughout the exercise. The EOF  ;' communications room contains dedicated telephone lines, commercial telephone lines and State Civil Defense radio for communications with the State and local EOCs.

telephones were the primary means of communications, backed up by other equipment as .. required. At 9:52 a.m. the exercise situation escalated to a Site Area Emergency. Shortly Q thereafter, in accordance with prior agreements within the locality, the EOC Director i , remmmended the evacuation of the Killona School. At 10:38 a.m., a precautionary ,

         ;              u- \ tion of the community of Killona was recommended and one siren in that
                           & anity was sounded (simulated). The news media was advised of this action and a                                             },

(sh , ated) EBS message was issued about this precautionary action. J: Following the decision by the utility to evacuate all non-essential personnel from , the plant site, the EOC Director decided to use his industrial hotline'to advise the petro- ', ! chemical industries within a mile of the Waterford 3 plant of the utility action and to suggest that they consider taking appropriate action. The verbal instructions from the q EOC Director to his communicator were misunderstood and the message transmitted j indicated that the EOC was recommending the evacuation of all non-essential workers. It is recommended that all outgoing messages be given to communicators in writing so ) that; a) the meaning of the message is clearly understood, and b) the identical message is i given to all of the many addressees on the hot line.

e. w A General Emergency was declared at 11:10 a.m. and the utility recommended,  ;'

t_ as a protective action, that the general public in evacuation sectors A1, B1, C1, D1, B2 C' and D2 take shelter. The utility communicator, at the TSC, who was on the hotline to r transmit this message seemed to lack comprehension of the meaning of the message and recommended that these sectors evacuate instead of shelter. It required considerable

      '          effort, by the other organizations on the hotline, to straighten out this misunderstanding.          '

i It is to their credit that they caught the problem and worked to resolve it. An " evacuation recommendation for sectors A1, B1, C1, D1,' B2 and D2 was issued by the F utility at 11:48 a.m. The EOC immediately initiated (simulated) all appropriate actions to deploy traffic control personnel and issued public announcements, via the EBS, cable j [. ' television and other media outlets, instructing the public on the actions that they should

   -i            take. The siren system was activated several times to alert the public of the EBS t          announcements that followed over the next few hours. At the same time instructions                     ,J were issued (simulated) to halt air, rail and water traffic into the EPZ. It was noted that

[ the decision to halt river traffic was not coordinated with the adjoining Parish (St. John ' ' t, the Baptist) until that Parish requested information on Coast Guard notification. It is recommended that the Transportation Officers Procedures be reviewed to insure

     '           coordination with adjacent Parishes in the event of a request to the Coast Guard to halt             .

[ Mississippi River traffic. 1; r- Throughout the exercise, instructions for the public were prepared by the PIO in *

coordination with the EOC staff. The PIO is the Assistant to the Parish Police Jury

    ]            President (Parish Chief Executive Officer) and empowered by law to act in his absence.               .

After consultation with the EOC Director, the draft messages to the public are read over

,[               a radio link to the EBS station where they 'are taped and then replayed to the public i                 (contact with EBS was simulated). The PIO also has direct access to all cable TV 4
      -          channels; could blank the screen and override the audio, or could directly input an

!<. emergency message on the government access channel (channel 11). All messages were also sent to the LOEP, LNED add the LP&L emergency news center. Information~ was (

r presented in terms of familiar landmarks as.well as the sectors. Schools, hospitals,

! nursing homes and industries were separately contacted via special communications links (simulated). The Parish dispatched a representative to the LP&L media center to serve as spokesman and liaison. His operations are covered in the media center portion of this I report. Although NOT an objective of the exercise, the initial public notification and protective action message drafting and dissemination (simulated) for the general

    ;             emergency took 20 minutes, slightly longer than the 15 minute federal standard. Also, f

At 2:05 p.m. the helicopter crew reported an ETA of 12 minutes to the hospital and provided the patient's status and vital signs to the Emergency Room staff. The helicopter arrived at 2:15 p.m. with the crew, the patient and an accompanying Health L. Physicist from the Waterford 3 plant. The patient and the helicopter were monitored for , 7 j' contamination and, while the patient was found to be contaminated, the helicopter was - clean and was released for other operations. The patient was moved into the prepared area of the Emergency Room for decontamination and medical stabilization. After ' passage of the patient, the helicopter landing area and the hallways were monitored for - contamination and found to be clean. By 2:37 p.m. the patient had been decontaminated,  ; medically stabilized and moved into the main Emergency Room for further medical P treatment. Hospital management and the hospital's PIO were advised of the patient's " ~ condition so that they could respond to requests for information if received. The ,, emergency room area and staff were monitored for contamination, dosimeters were read, '~ . and then protective material and clothing were removed and the area returned to normal. ' 1
