HIGHLIGHTS OF WEEKI.Y INFORMATION REPORT WEEK ENDING MAY 23, 1986 Seabrook Unit 1

New Hampshire Yankee (NHY) announced on May 14, 1986, that Seabrook i Station is expected to meet its fuel loading schedule date of June 30, 1986, and would be ready for commercial operation on October 31, 1986. However, delays in the licensing process (Emergency Preparedness) will prevent commercial operation on the scheduled dated.

NHY noted that because Massachusetts Governor Dukakis has not filed Massachusetts' emergency plans with FEMA, NHY will not be able to begin commercial operation as scheduled by October 31, 1986. NHY stated that it was not likely that they would be able to complete the licensing process anc ha've the unit in commerci.al operation until spring of 1987.

B&W Owners Gro@ Report .

On May .16, 1986, the B&W Owners Group (BWOG) submitted a two-volume report cescribing the BWOG Trip Reduction and Transient Response Improvement Program. This program was developed in response to the January 24, 1986 EDO letter noting the need for a reassessment of the basic design requirements for S&W plants. The BWOG program was conttructed to addru s the frecuency and complexity of post-trip respense in B&W plants. Tne SWOG plan isc 'u'crently undergoing staff review and will be incorporated into the stoff's overall plan for reassassing B&W plants.

0FFICE OF NUCLEAR REACTOR REGULATION ITEMS OF INTEREST Week Ending May 23, 1986 Daily Highlight On May 15, 1986, a full-power license was issued to Duke Power Company for Catawba Unit 2. ;urrently, the unit is at 2 percent power. Power generation is expected within the next day.

In a separate event, Unit I had a trip from 100 percent power on May 14, 1986, as a result of a steam generator level transmitter failure. Duke plans to replace the transmitter and the power supply card, and expects that the unit will be back on line by midnight, May 15, 1986.

Pilgrim Nuclear Power Station At midnight (24:00 hours), October 15, 1986, a strike of union members at Boston Edison Company commenced on a company-wide basis. The reason for the strike was hilure to agree on terms for renewal of the union-company contract which expired at that time.

Pilgrim's control room is being staffed by senior reactor operators for the duration of the strike. The plant is shutdown for repairs but is expected to restart next week if the Regional Administrator so authorizes.

FERMI-2 The staff been informed that four return springs in two of the Fermi-2 MSIV's were found to be broken. All eight of the MSIV's failed their LLRTs within the last two weeks, and the licensee was examining two of the MSIV's to determine the cause of this gross leakage. Since the four springs are from the same metallurgical heat, there is a question concerning the reliability of the other springs in the MSIV's. Region III advised the staff that the licensee's own research lab has determined that these springs developed cracks during quenching. A Part 21 notification is presently being evaluated by the licensee. The vendor for these MSIV's is Atwood-Morrell. The vendor for the springs is Duer Spring and Manufacturing Company.

Palo Verde Units 1 and 2 Palo Verde Unit 2 is in Mode 3 (HOT STANDBY) and is expected to enter Mode 2 (START UP) on the evening of May 16, 1986. The unit is expected to reach its initial 5% power on May 17, 1986. Unit 1 is in Mode 5 (COLD SHUTDOWN) and is expected to remain in that mode for the next two days. With respect to May 14, 1986 event related to the interruption ENCLOSURE B MAY 2 31906

. of the offsite power transmission lines, all three lines were restored as of 5:00 pm, PDT, May 15, 1986. One offsite power transmission line was intact at all times and the emergency diesel generators for both units were tested and found to be operable on May 15, 1986. The FBr is investigating the event.

Unanticipated Reactivity Addition (One Dollar Plus) in the Texas A&M TRIGA Reactor On May 5, 1986 the staff was contacted by Region IV which had been informed on May 2nd by Texas A&M officials that their TRIGA reactor (pulse type) had experienced an unexpected reactivity addition on May 1.

The unexpected reactivity addition (1.09 dollars) occurred as a result of removing an experiment of, apparently, unknown worth. The experiment was removed while the reactor was operating approximately 550 KW(t). Upon removal of the experiment the reactor was scrammed automatically by its protection system (high flux trips). It appears that no safety limits were exceeded, nor was there any damage.

On May 6th, Region IV personnel went to the site on a fact-finding visit. We plan to meet with the licensee's management to discuss the root causes and corrective actions to preclude recurrence of such events.

ENCLOSURE B MY 2 31986

. Catawba Unit 2 On May 17, 1986, at 7:55 a.m., Catawba Unit 2 tripped from 15 percen.t power on 10-10 level in the 2A steam generator. It returned to criticality at about 8 p.m. the same day.

On May 18, 1986, at 9:47 a.m., the main generator was lined up with the grid and producing its first electricity. At 11:24 a.m. the operator manually tripped the reactor on hi-hi level in the 2D steam generator.

On May 19, 1986, at 4:20 a.m., the reactor returned to criticality. At 2:10 p.m., the Unit 2 reactor tripped on 10-10 level in the 2A steam generator.

Catawba Unit 1 On May 16, 1986, at 1:43 p.m., the Catawba Unit I reactor returned to criticality. On May 17, 1986, at 2:38 p.m., the reactor was at 44 percent power when two fuses failed in the rod control cabinet. While Duke's maintenance personnel were in the process of replacing the fuses, they placed too many control rods on D. C. hold bus. This caused negative rate flux and resulted in reactor trip. The reactor returned to criticality at 4:28 a.m. on May 18, 1986. Currently, it is at 100 percent.

River Bend Station At about 7:00 a.m. CDT on May 19, 1986, while River Bend Station was at 75% rated power, the fire control system water spray in the turbine building was activated. This water spray was directed into the housing surrounding the turbine bearings. The cause for the water spray activation is currently unknown; however, the licensee found no electrical problems in the water spray system, but controls for manual activation of the spray are located in an unsecured area. The water spray was active for about 15 minutes until secured.

At 2:30 p.m. CDT, the River Bend Station's turbine automatically tripped on high vibration which in turn triggered a reactor scram. The licensee subsequently determined that two turbine vibration probes had filled with water, probably from the earlier water spray on the turbine bearing housing area.

The reactor was restarted at 1:00 a.m. CDT, May 20, 1986. At 3:00 p.m.,

the reactor was at 55% rated power, with no unusual vibrations observed in the turbine.

This scram was the 10th inadvertent scram at the River Bend Station in 1986.

ENCLOSURE B

-4 Limerick

, On May 19, 1986, the staff was informed t,y the licensee that testing of 58 of the excess flow check valves was performed during power operations in the three weeks prior to the shutdown of the plant on May 2. This was in contrast to our understanding of statements made in the application for license amendment of December 18, 1985, wherein the licensee stated that the entire group of 119 valves could not be tested while at power.

The amendment has been addressed by both a Licensing Board and an Appeal Board and is now before the Commission. The licensee's counsel has informed the Secretary of the Commission of this situation by letter, dated May 19, 1986.

The staff does not believe that this new information invalidates the license amendment, which was issued on February 6, 1986, especially regarding the remaining 61 valves which could not be tested at power.

4 However, it does reflect on the adequacy of the licensee's procedures for notifying the staff of revisions in previously submitted information.

Byron 1 Byron 1 was derated to 75% power at approximately 1:00 am, EDT, on May 18, 1986. The utility took this action based,on Westinghouse's recommendation that reducing the hot leg temperature would decrease the potential for stress corrosion cracking on the primary side of the steam generator hot leg tubes. Although Byron 1 has not had any leaking tubes attributable to this phenomena, this action is being taken as a preventative measure.

The utility will provide additional information in a meeting that will be held in the near future.

4 NRC TMI-2 CLEANUP PROJECT DIRECTORATE WEEK ENDING MAY 23, 1986

1. DEFUELING -

- Pick and place defueling activities resumed on May 23, 1986. The number of canisters transferred to the fuel pool remains at 42.

- The second of four batches of the Reactor Coolant System (RCS) draindown and refill process was completed on May 22, 1986.

- Batch three of the RCS draindown and refill will start late on May 23, 1986 with refill to be completed on May 24, 1986. Defueling  !

operations will be performed on May 25, 1986 and the fourth batch of drain and refill will be performed next week.

- Both of the defueling work platform jib cranes have been modified and returned to service.

2. PLANT STATUS ,

- The reactor remains in long term cold shutdown, vented to the atmosphere. Core cooling is by natural heat loss to ambient building atmosphere. The average incore thermocouple reading is 89 F. Water visibility is approximately 1 foot.

- The airborne radioactivity on the defueling platform is about 7.0 E-11 uCi/cc Tritium and 2.7 E-7 uCf/cc particulates, predominately Cesium-137. The platform is mounted above the modified internals indexing fixture which is mounted over the reactor vessel. These provide about 15 feet of water over the core region and 6 feet over the carousel holding the defueling canisters.

3. WASTE MANAGEMENT The Submerged Demineralizer System and EPICOR II are in temporary shutdown.
4. ENVIRONMENTAL MONITORING

- Results from US Environmental Protection Agency (EPA) analysis of the NRC outdoor air sampler for the period May 8 - 15, 1986 are:

Prefilter I-131 - 2 1 E-14 uC1/cc Cs-137 - 1.9 0.6 E-14 uCi/cc Charcoal Filter I-131 - 8.5 4.2 E-13 uCi/cc The same sampler for May 15 - 22, 1986 resulted in concentrations of Cs-137 and 1-131 below the analysis lower limit of detectability of 1.1 E-13 uCi/cc. The results for the May 8 - 15, 1986 period are consistent with those obtained from other samplers in the area and the State, and are attributed to fallout associated with releases from the Chernobyl reactor.

4. ENVIRONMENTAL MONITORING, cont'd.

- EPA sample analysis results show that TMI site liquid effluents are in accordance with regulatory limits, NRC requirements, and the City of Lancaster Agreement. -

- TMI water samples taken by EPA at the plant discharge (includes TMI-1 and TMI-2) to the river consisted of seven daily compasite samples taken from May 3 - 10, 1986. A gamma scan detected no reactor related activity above the lower limit detectability.

- The Lancaster water sample taken at the water works intake and analyzed by EPA consisted of a seven day composited sample taken from May 4 - 10, 1986. A gamma scan detected no reactor related radioactivity.

5. AUXILIARY AND FUEL HANDLING BUILDING ACTIVITIES ,

- Preparations are being made for cleaning up the highly contaminated auxiliary building sump room. A gross water flush will reduce the contamination levels to provide easier access.

- Assembly has continued on the desludging system which will be used first on the neutralizer tanks and then on the auxiliary building sump.

An acceptance testing program is being done for the rail car mounted shipping casks. The testing involves all the components of the system which removes core shipping canisters from the "A" Spent Fuel  ;

Pool and loads them into the cask. The testing program will also be '

conducted on the system which removes the cask from the rail car and ,

lifts it from the horizontal shipping position to the vertical loading position.

On May 21, 1986, G. H. Bidinger net with B&W representatives to discuss some aspects of the characterization plan for the Apollo site. Discussions were focused on gridding of the site outside the buildings, radiation surveys of the outdoor area, surface soil sampling and analysis of select grids, soil core and soil sampling, and analysis of soil under select locations under the facility. The surface soil samples will be taken in grids which are selected at random and in grids which are bias selected because of radiation levels 3 above background (assuming such levels are identified in the radiation i

surveys) . The core samples will be taken at biased locations in the grids and inside the building. The biased locations will be based on radiation 4 measurements (in outdoor areas) and in solution process areas inside the building, B&W will continue to develop the plan for further discussion with

.; the NMSS staff.

Paducah Gaseous Diffusion Plant On May 13-14, 1986, W. T. Crow and W. S. Pennington visited the Paducah Gaseous Diffusion Plant to tour the facility and discuss their safety review and documentation programs for UF 6 handling.

(3) Southern Company Services, Inc., Birmingham, AL - a first time inspection to evaluate engineering efforts for the Hatch and Farley nuclear plants, Part 21 evaluation and reporting procedures, computer code error handling procedures, and the procurement process for safety-related components.

c. Quality Assurance

} Members of the Quality Assurance Branch are participating in ASME I Nuclear Quality Assurance Comittee meetings in Denver, C0.

d. Emergency Preparedness i (1) A FEMA emergency preparedness exercise report for Grand Gulf was forwarded to the region for appropriate action. FEMA identified three deficiencies and is requiring that a remedial exercise be conducted by June 4, 1986.

Items of Interest Week Ending May 23, 1986 Direct Containment Heating (DCH)

On May 14, 1986 the DCH-1 test was conducted at Sandia National Laboratory in Albuquerque, NM. DCH-1 is the first test to be conducted in the SURTSEY facility that has been constructed at SNL to investigate the effects of high pressure melt ejection / direct containment heating. A 1:10 scale model of Zion reactor cavity was placed in the SURTSEY vessel together with the melt generator so that the debris discharged from the apparatus entered directly to the contained atmosphere.

DCH-1 is intended primarily for shakedown of the facility and, as such, test conditions were conservatively selected to ensure that the design limits of the vessel would not be approached by the induced pressure and temperature of the atmosphere. 20 kg of the melt were used in this test as compared to 80 kg that are planned for subsequent tests. The injection pressure was 390 psig, while a range of pressure from 600 to 1500 psig are planned for future tests.

The observed pressure rise of slightly over 1 bar agreed reasonably well with the pre-test prediction by the CONTAIN code. Preliminary indication of aerosols, however, seems to suggest that the mass of aerosolized materials may significantly exceed the predicted amount of 4 percent by weight.

The SURTSEY tests are expected to provide NRR with a data base for evaluation of the risk associated with direct containment heating.

Subsequently, by the attached letter to Chairman Palladino from David Ward, dated May 13, 1986, the ACRS formally requested Commission financial suppprt for the above mentioned organizations. The Chairman transmitted this letter to the EDO for action.
