APPENDIX B NOTICE OF DEVIATION Kansas State University Docket: 50-188 Kansas State University Nuclear Reactor Facility Operating License: R-88 Based on the results of an NRC inspection conducted on March 8-10, 1988, a deviation from your Hazards Summary Report was identified. The deviation consisted of failure to record entries as required by the Hazard Summary Report,Section II.7.2. In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1987),
the deviation is listed below:
Failure to Properly Document The Hazard Summary Report, Section 11.7.0, Operations ' ogbook, Step 2, states "All scrams and their cause as well as malfunctions of equipment must be entered in the logbook in red."
In deviation from the above, the licensee was found to have highlighted in red only one of ten equipment malfunctions, between the dates May 1986 through March 8, 1988. (188/8801-01)
Kansas State University is hereby requested to submit a written statement or explanation to this office within 30 days of the date of the letter transmitting this Notice. This reply, should include for each deviation:
(1) the reason for the deviation if admitted, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further deviations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration' will be given to extending the response time.
