APPENDIX A' NOTICE OF VIOLATION Kansas State Univer:1ty (KSU) Docket: 50-188/86-02 TRIGA MARK II Reactor License: R-88 During an NRC inspection conducted on September 9-10, 1986, violations of NRC requirements were identified. These violations included failure to perform survey instrument calibration at required' frequencies, failure to develop written procecures, and failure to perform surveys. In accordance with the

" General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1986), thd violations are listed below:

Failure to Perform Survey Instrument Calibrations A.

10 CFR 50.54(r) requires test and research licensees to submit emergency plans.

Section 10.5 of the KSV Emergency Plan dated October 14, 1982, states, in

'T part, that portable survey instruments will be calibrated semiannually.

Contrary to the above, the NRC inspector determined on September 9, 1986, that the licenseeihad not performed semiannual calibration of the following portable survey instruments: (1) Eberline Model E-120, S/N 915 i during' the period June 1984 to July 1985, (2) Eberline Model E-120, S/N 940, during the period January to December 1985, (3) Eberline Model E-500B, S/N 968, during the period January to December 1985, (4) Eberline Model E-5008, S/N 1021, during the period January to September 1985, and (5) Eberline Model E-510, S/N 153, during the period July 1985 to June 1986.

B. Failure to Develop Written Procedures Technical Specification, Section H, Administrative Requirements, states that written instructions, approved by the Reactor Safeguards Committee, shall be in effect for, but not limited to, specific area noted in Section H.

Contrary to the above, the NRC inspector determined on September 9-10, y 1986, that written instructions were not in ef fect for calibration and um operation of the: gamma spectroscopy system, the TLD personnel monitoring Oo$ system, and the liquid scintillation counting system.

, Contrary to the above, the NRC inspector determined on September 10, 1986, that the licensee had not performed surveys to: (1) evaluate liquid effluents for alpha activity prior to release to unrestricted areas, and (2) analyzed reactor bay atmosphere to determine if airborne concentrations exceeded 10 CFR Part 20 limits.

This is a Severity Level IV violation. (Supplement IV) (188/8602-03)

Pursuant to the provisions of 10 CFR 2.201, Kansas State University'is hereby required to submit to this office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation: (1) the reason for the violations if admitted, (2) the corrective steps which have been taken and the results achieved, (3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
