This is a transcript of the telephone conversations of the Headquarters Opera-tions Officer concerning the Davis Besse incident that took place on June 9, 1985. These conversations took place between the hours of 0211 through 0851.

Although every effort has been made to accurately transcribe the original taped conversations, this may not be a verbation text due to low volume, interfer-ence, or unintelligible recordings.

(B) We just recently had a trip. We had a main feed pump trip; we had a com-plete aux feed pump trip. We've got the aux feed pumps back now and the plant appears to be stable.

(A) Yeah, what was the cause of the reactor trip?

(B) It's undetermined right now, but the MSIV's both closed and as far as I know that's... that's what we know as far as the initiating event, at the moment. The shift supervisor might know more; I don't.

(A) Okay, I'll need to know what would be the cause of the reactor trip once you determine it.

(A) Okay; so you said that you: main feedwater pumps tripped?

(B) The main feedwater pumps tripped; the aux feedwater pumps either failed to initiate or tripped. At any rate, they weren't running when I came in.

(A) Okay, have you determined what the cause of that problem was?

(B) What's that?

(A) Have you determined what the cause of the aux feedwater pump trip is?

(B) No, we haven't.

(A) Okay, is that still under investigation?

(B) That's still under investigation. They seem to be, seem to be controlling okay right now though, manually, s0.....

(A) Okay. And what was the cause of the main feedwater pumps tripping? Do you know?

(B) That we don't know. We had been having trouble with them before and we have a whole lot of the instrumentation attached there. It will be hours before we get to investigating that, so... We also don't have, we broke condenser vacuum because of drop in steam header pressure while we were trying to get back off the aux feed pump.

(A) Anything else unusual or not understood?

(B) Not anything that I can really think of right now. I gotta get your name now.

(A) I need to know why you're notifying me. Are you declaring an Unusual Event?

(B) Yeah. We.'do have an unusual event.

(A) Okay, that is very important, I need to know that. Okay, what time did you declare this an unusual Event?

(B) Hold.that. We haven't declared an Unusual Event. We are just in the pro-cess of evaluating it.

(B) The plant appears to be stable right now. I'll get back to you as soon as I find a little more on this.

(C) Yes.

(A) Davis Besse took a reactor trip at 0133.

(C) Okay, let me get to the other phone so I can take this information down, okay?

(B) And exactly what happened after that I still can't tell you too good. .

We're just working on figuring everything else out.

(A)-Okay. And so, could you give me a call back once you have, determined the cause of the feedwater pump trip? And also we're interested in the aux feedwater pump trips also.

(B) Yeah, like I said. I'm not sure whether that was a trip or a failure to initiate, but at any rate they wanted to--it didn't come on by itself, s0.....

(A) Okay, the cause of the reactor trip appears to be the main feedwater pump No. I tripped.

(C) Did they just call you back?

(A) Yes, they did. Evidently the person who was talking to me is real disori-ented. In fact, they know very little about this event it seemed like.

.now, and they're operating normally.

(C) So they tripped and then they restarted them evidently.

(A) Well, this person appears did not know the information.

(C) Did they trip? Do we know that for sure?

(A) No, we don't.

(C) Okay. I'll just put down ' tripped?' for right now then.

(A) Right and once that guy gets back to me, apparently they're scurring around to see what's going on with this, but I will have a good nice talk to him once he calls me back again. Make sure we get all these questions

(A) Evidently this person must have been the operator, STA, or something talk-ing to me; you know the other people are busy.

(C) Okay, so the cause was a feedwater problem, then.

(A) Right.

(C) Okay, say that their main steamline isolation valve closed on 'em...

(A) Okay.

(C) Other than that, that's about all I know about_at this point. The reactor was tripped from 90% power, now it's in Hot Shutdown; stable.

l (B) We do have an Unusual Event at least now. At 0225.

l (A) Okay, 0225. Okay, and repeat, is the plant stable?

(B) What's that?

(A) Plant stable right now?

(B) Yes, we're stable right now.

(A) Okay. What's the reason why you declared an unusual Event?

(B) Basically, I guess, we would have declared earlier on an Alert based on loss of--a Site Area Emergency based on the loss of all feedwater, but since it was quickly restored before we could--within a few minutes before we could get into the emergency plan, we're not really in any obvious sec-tion of the emergency plan, but since we're uncertain as to what the cause l of the loss of initiation or the lack of initiation of aux feedwater was, and since the main feedpumps are tripped still, the Shift Supervisor's a little uneasy about things and that's why he wants some emergency plan activation.

(C) Okay, due to the questionable operation of the aux feedwater pump.

(C) Okay. Other than that the plant is still stable,'and they're still inves-tigating the cause of the... either the pump tripped or failed to start.

(A) Either it tripped or it failed to start.

(C) .Right.

(A) Okay, very good.

(A) Okay, Davis Besse declared an Unusual Event due to a reactor trip and a bunch of weird things happened after the reactor tripped. The reactor trip was caused by a main feedwater pump tripping, which then isolated the main steam isolation valve. The problem arises'when the aux feedwater pump failed to--either failed to start or---

(D) Whoa, whoa, whoa, hold on. The main feed pump tripped and that isolated the main steam isolation valve?

(A) Pardon me?

(D) The feedwater pump tripped and you said that automatically isolated the main steam isolation--

(A) Hey, I didn't say 'automatica11y'. That's what they told me.- I still--

that's one question I gntta ask them. Probably it...it's usually designed to isolate with less than 600 lbs of steam pressure.

(D) Right, it's a safety va'.ve.

(A) And so, if you trip a pump.

(D) If you trip a pump the pressure goes up. Alright, go ahead. What else?

(A) Anyway, the problem arose when the aux feedwater pump either failed to start or it imediately tripped.off after starting. But it had to be ini-tially manually initiated. Okay. And that occurred for about 5 minutes and they started their pumps up. And so the plant is stable, everything is normal right now. The aux feedwater pump is still operating as normal but the cause of the problem on the aux feed pump is still undetermined at this time.

(D) And so the plant is' stable.

(A) Uh, yes.

(D) Okay. And does the Region III Duty Officer know?

(A) Uh, yes he does.

(D) Okay, fine. Okay, thanks for calling, (name deleted).

(A) Okay, good'enough.

(E) Yes. Hi. (Name deleted) from RIII. I just got a call from my resident at Davis Besse...

(A) Yes.

(E) That they've had a trip.

(A) Right.

(E) And, that he was going to go out and see what the devil was going on. He didn't get any details, but they'd gotten themselves in a tight fix.

(A) Yes.

(B) What did the licensee tell you?

(A) Okay, now who's this speaking, please?

(E) This is (name deleted). I'm a section chief...

(A) Okay, (name deleted). They called me up on a reactor trip.

(E) Right.

(A) Let me get my sheet out. Okay, the cause of the trip, which they told me on the second phone call, was due to main feedwater pump No. I tripping.

(E) Right, and they had the problem with the auxiliary feed pumps.

(A) Right. And they also had a main steam isolation valve isolation. The biggest problem was the aux feedwater pump had to be manually started be-cause it either failed, or failed to start, or tripped. And that cause is still under investigation.

(E) Did they declare .it an Unusual Event?

t (A) Yes, they did. This was after I questioned the fact that the person who was talking to me knew very little information, and it took a third phone call.in order to declare an Unusual Event. I asked him what the event 3

classification was, and he said at that time he didn't know.

02:55:00 (E) I was just looking in our NRC book and it says that one of the initiating conditions for notification of an Unusual Event would be loss of engi-neered safety feature or fire protection system function requiring shut-down by technical specification. I would say that any danger of losing auxiliary feedwater would automatically qualify as an Unusual Event.

(A) Right. But in their emergency plan the loss of the aux feedwater system is declared a site area emergency.

(E) Oh? But did they say they declared a site area emergency?

(A) No, they didn't. Okay, within a couple minutes they' started the pumps, and by that time they weren't even in their emergency plan.

(E) Is the plant stable?

. .. - 4 s (A) Yes. The plant is stable and the aux feedwater system was operating prop-erly within, I think, five minutes, they mentioned, after the reactor trip.

(E) Well, anyway I thought I'd call to let you know that (name deleted) and (name deleted) both probably be there within a half hour to forty five minutes.

(A) That's probably a good idea because the person I talked to knew very lit-tie information, and they seemed pretty disoriented when I talked to him on the phone.

(E) Right, I think, did you tell the Region III Duty Officer.....

(A) Yes, I did.

1 (A) Pardon me?-

(E) When did you call him? We need to follow up. I'd like to suggest you call him back when I hang up.

(A) Okay. I called him about 15 minutes ago.

(E) Oh, woke him up to take the call?

(A) Right.

., 'u e The next conversation occurs at 0511 when the Operations Officer calls the plant for the routine morning status report.

A = Headquarters Operations Officer B = Licensee (B) Hello, Davis Besse.

(A) Yes, good morning, your status.

(B) Okay. Right now we have the startup feed pump running and it's supplying

feedwater to both steam generators. We have both aux pumps, feedwater pumps available but not pumping water. Right now we're in the process of reestablishing vacuum in the condenser and normal post-trip lineup.

, (A) Okay. Any new news on all the trips?

(B) All what trips?

(A) Oh, the cause of the main feedwater...

(B) Oh, okay. No, there's no... we've got some chart recorders that have data on them. We're analyzing it. The feed pump did trip and appeara to be on a... well, we've got indication that it may have been on overspeed.

(A) Okay.

(B) We have no... we do understand we had a what we call a steam and feedwater rupture control system trip concurrent with the feed pump trip. And that was what isolated... did the isolation and secured the other main feed 4

pump.

(A) Oh, okay.

'1 (B) And then both aux feedwater pumps started but tripped due to problems with their trip throttle valve.

. (A) 0kay. So you're telling me the trip throttled this valve?

i (B) We originally thought it was overspeed because that's the only thing that

. really trips them but we're, we're thinking, we're having problems with

- the trip throttle valve. And once they were ...the trip throttle valves were reset they've been running. We've had one system that has not, one auxiliary feedwater system that we've declared inoperable, although it's available; we just don't have the proper control.

(A) Okay. You still in your Unusual Event?

! (B) Be're still on the unusual event. We could get out of it, but we'd like to stay in it until we get vacuum restored and get off of our safety systems.

(A) Okay. It sounds pretty good.

(B) Okay.

(C) Okay. Let me get down to my other phone.

(A) Okay.

[ PAUSE, WHILE PHONES ARE SWITCHED]

(C) Okay.

(A) Okay. I called back Davis Besse during the morning status calls, and they cleared some information for me. Okay, the aux _feedwater pumps apparently had problems with the trip throttle valve.

(C) Say that again?

(A) The aux feedwater pump...

(C) Okay, aux feedwater pump...

(A) Right. Apparently had some problems with the trip throttle valve.

(C) Okay. Problems with trip throttle valves.

(A). Right. And they suspect that to be the problem of why it tripped off. But

they're still investigating.

(C) Okay.

(A) Okay, the MSIV closure, OK, is still a mystery. But, apparently it was actuated by the feed steam leak isolation system.

(C) Say that again, now?

(A) Okay, the cause of the main steam isolation closure was a result of the signal from the feedwater main steam leak isolation system.

(C) You're breaking up a little bit, I'm sorry. Try again.

(A) Okay. The reason why the main steam isolation valve closed was due to apparent signal from the feedwater steam main steam isolation signal.

(C) Okay.

(A) But it's still a mystery why it actually actuated.

(C) Okay.

(A) Okay,-that resulted in a tripping of the other main feedwater pump. So therefore they lost all main feedwater. Remember that the whole event initiated from the tripping of the No.1 main feedwater pump...

(C).0kay.

-(A) ...cause 'still unknown. At the same time the main steam isolation valve closed on 'em.

-(C) All right.

.. o g (A) It is still under Unusual Event when I talked to them a couple hours ago; they will remain'so until they regain condenser vacuum.

(C) Okay.

(A)' Let me see. Other than that, that's about all I have for the Davis Besse event.

(C) Okay. The supervisor told me, after we talked, he and I talked, and he sent both .the resident and senior resident to the site. As soon as they get out there and get this thing sorted out, he told me-after he got a complete update a little later on this morning, probably around 8:00 o' clock our time, he's going to call me and give me a complete rundown.

(A)- Okay, sounds good.

(C) Okay?

(A) Okay, thanks.

(C) ' Bye.

, (A) No problems. At Davis Besse, they're still in an Unusual Event and will remain so until they restore condenser vacuum.

l (D) Okay. They're still working on aux feed.

j (A) Right. And they suspect--

I f (D) I didn't realize they had lost condenser vacuum. Oh, but they had the main steam lines isolated.

(A) Right. And the cause of that's still unknown. But it happened at the same time when they lost the No. 1 main feedwater pump that started the a

whole show.

(D) Okay, so they're still cooling with aux feed.

(A) Right.

(D) Are they dumping to.....?

(A) Hold on, (name deleted), no they're not cooling with aux feed. They se-cured aux feed; they restored feedwater pump. The start up feedwater pump.

-(A) Yeah, yeah, they're dumping that in the atmosphere, of course.

(A) Right. They thought it was overspeed but that's the only thing that can trip th'ese things, but they're lo'oking at it a little closely, and they think that it could be a problem with the trip throttle valve. But when they reset it, everything was fine.

(D) Okay. Why? What was the problem with the condenser vacuum that they didn't get the main steam isolation valve open?

(A)- It takes some time to pump these things out. This was about an hour and a half ago during morning status, about 2 hours2.314815e-5 days 5.555556e-4 hours 3.306878e-6 weeks 7.61e-7 months ago, when I talked to them.

(A) So, other than that'everything is normal.
