Title	Agreement State Report - Source Failed to Retract
Event Description	The following information was received from the Florida Bureau of Radiation Control (the Bureau) via email: On 3/2/2020 at 1850 EST, [the licensee's Radiation Safety Officer (RSO)] called to report that a radiography source uncoupled from its drive cable, but remained in its sleeve during an x-ray of a processing tank. The time of the incident was approximately 1630 EST. On site materials were used to shield and secure the source which was subsequently locked in the licensee's storage facility. No abnormal or excessive doses were reported. [The licensee] will provide a written report to the Bureau within 10 business days. The gauge is a QSA Global model 880 Delta (S/N: D4132) with a 76 Ci Ir-192 source (S/N: 94135G). Florida Incident Number: FL20-025
