2. Report Date: 3/15/77 3. Occurrence Date: 2/19/77 4 Facility: Arkansas Nuclear One-Unit 1 Russellville, Arkansas
5. Identification of Occurrence:

Failure of the High Pressure Injection Flow Transnitters (PDT-1210 6 PDT-1209) on Loop "A" and the Flow Transmitters (PDT-1228 6 PDT-1230) on Loop "B" being out of manufacturer's tolerance.

6. , Conditions Prior to Occurrence:

Steady-State Power Reactor Power 0 MWth Hot Standby Net Output 0 MWe Cold Shutdown Percent cf Full Power 0 g Refueling Shutdown X Load Changes During Routine f) Pcner Operation V Routine Startup Operation Routine Shutdown Operation Other (specify)

7. Description of Occurrence:

At 1400 hours0.0162 days
0.389 hours
0.00231 weeks
5.327e-4 months
on February 19, 1977, while performing a surveillance test on the HPI flow transmitters, it was discovered that the two flow transmitters on Loop "A" (PDT-1210 and PDT-1209) had failed and the two flow transmit.ters on Loop "B" (PDT-1228 and PDT-1230) were out of the recommended manufacturers tolerances. -

The bellows of. PDT-1210 and PDT-1209 had failed, therefore the flow indicator would not have indicated flow upon the actuation of an ES signal. PDT-1228 would have indicated flow 1% low and PDT-1230 m would have indicated 4% low upon actuation of an ES signal.

8. Designation of Apparent Cause of Occurre- e:

Design Procedure Manufacture Unusual Service Condition Including Environmental Installation /

Construction Component Failure (See Failure Data)

Operator Other (specify) X O'ne bellows appeared to have failed due to fatigue and the other bellows appeared to have failed due to being over pressurized. The

, reason for two transmitters being out of tolerances is unknown. l k 9. Analysis of Occurrence:

Since both HPI loops remained operable even though the transmitters were inoperable or inaccurate, there was no hazard to the health to the health and safety of the public.
