Identification of Occurrence:
Failure of the High Pressure Injection Flow Transnitters (PDT-1210 6 PDT-1209) on Loop "A" and the Flow Transmitters (PDT-1228 6 PDT-1230) on Loop "B" being out of manufacturer's tolerance.

Description of Occurrence:
At 1400 hours0.0162 days <br />0.389 hours <br />0.00231 weeks <br />5.327e-4 months <br /> on February 19, 1977, while performing a surveillance test on the HPI flow transmitters, it was discovered that the two flow transmitters on Loop "A" (PDT-1210 and PDT-1209) had failed and the two flow transmit.ters on Loop "B" (PDT-1228 and PDT-1230) were out of the recommended manufacturers tolerances.
The bellows of. PDT-1210 and PDT-1209 had failed, therefore the flow indicator would not have indicated flow upon the actuation of an ES signal.
PDT-1228 would have indicated flow 1% low and PDT-1230 m
would have indicated 4% low upon actuation of an ES signal.

X O'ne bellows appeared to have failed due to fatigue and the other bellows appeared to have failed due to being over pressurized.
The reason for two transmitters being out of tolerances is unknown.

Analysis of Occurrence:
Since both HPI loops remained operable even though the transmitters were inoperable or inaccurate, there was no hazard to the health to the health and safety of the public.

1 The two bellows were replaced in PDT-1210 6 PDT-1209 and adjustments were made to PDT-1228 6 PDT-1230.
