,ENCLOSURE 4 February 24, 1988 Salem Unit 1 - Steam Generator Eddy Current Examinations Technical Specification 4.4.5.5.b. - Steam Generator eddy current reporting.
Examinations were conducted on No. 13 Steam Generator and were performed by Westinghouse NSID, coordinated by the Salem ISI Group, during the 7th refueling/10 year ISI outage.
Results of the examinations are as follows:

(*) Sxl testing was performed on 406 ~ubes. Of the 406 tubes, 122 were also inspected using the standard bobbin coil probe and were included in the count of full length/u-bend tubes inspected per tech spec.
Therefore, only 284 tubes will be added to the total number of tubes inspected.
Three (3) tubes were plugged as a results of the examinations.
Two (~) tubes were plugged because they exceeded the plugging limit per technical specifications and one (1) tube was plugged per Westinghouse recommendation for not having AVB contact.

ENCLOSURE 4 February 24, 1988 Current Status of the Steam Generators At this time the total number of tubes plugged in the Salem Unit 1 Steam Generators are as follows:
Generator # 11 12 13 14 Tubes Plugged 14 18 16 31 NOTE: In the Steam Generator's 11-14, the first five (5) and the last Five (5) tubes in row 1 were mechanically plugged because of a generic wearing problem on the tubes, due to the vibration of the tube lane blocking device.

Bxaminations were conducted on No. 24 Steam Generator and were performed by Westinghouse NSID, coordinated by the Salem ISI Group, during a during a forced outage in November 1987.
Results of the examinations are as follows:
