The inspectors determined that additional inspection was required for the inactive leak on the heater
sleeve penetration in the pressurizer bottom head.
Technical Specifications 3.4.13 RCS Operational Leakage, states that
RCS operational leakage shall be limited to no
pressure boundary leakage in Modes 1 through 4. Contrary to this requirement, on February 25, 2008, Constellation identified
RCS deposits indicated that the
sleeve welds had been leaking during the previous operating cycle. The penetration
sleeves are part of the
RCS pressure boundary. The leak was discovered during scheduled visual examination of the pressurizer head penetration area. Constellation entered this issue into their
CAP (CR IRE-029-507) to complete a cause analysis and reportability review. An unresolved item (
URI) has been opened to track this issue pending review of Constellations evaluation to determine if a performance deficiency exists. (
