Request for additional informatio n regarding NRC License No. SNM-1826
Dear Ms. Kuhls:
We have reviewed your amendment request dated August 23, 2018. In the letter, you request to name Carl Jindra at an authorized user for all licensed materials. We determined that the information regarding training and experience for Mr. Jindra did not cover all of the criteria for acceptable training and experience for authorized user discussed in NUREG-1556, Vol. 17, Rev. 1, Appendix M.
Please provide additional information describing Mr. Jindra's training and experience that he has completed the radiation safety training and passing the test as discussed in the NUREG above, including, but not limited to, the training with your operating and emergency procedures, license conditions, NRC regulations (10 CFR Parts 19, 20, and 70, and 30, 31, 32, and 71, as applicable), and storage, control, and dispose of licensed materials. In addition, please provide more details of his hands-on training, such as, the isotopes use, physical
form,quantities handled, activities performed, duration of experience, and has passed the practical examination assessed by the RSO.
