No outage activities were in progress; DAEC did not have a refueling outage in 2011. No fatigue declarations
were received. No management actions arose from the assessments.
No corrective actions were performed as a result of the waiver or fatigue assessments.
The conclusion for all fatigue assessments was that the involved personnel were not fatigued.
The waiver required on 3/3/2011 due to a relief calling in sick when no other replacement was readily
available was considered to be an isolated occurrence, but such occurrences are trended.
