ENS 5642219 March 2023 18:12:00The following information was provided by the Louisiana Department of Environmental Quality (the Department) via email: On March 19, 2023, Versa Integrity Group, Inc. notified the Department that there was a possible overexposure. The industrial radiography crew was working with a QSA 880 Delta (source projector) with 100 Ci of Ir-192 at Air Liquide, Geismar, LA. The camera serial number is D14952. The Ir-192 source serial number is 72308M. The crew pocket ion chamber went off scale. The industrial radiography source is secured and properly shielded within the QSA 880 Delta camera. The following additional information was obtained from the licensee in accordance with Headquarters Operations Officers Report Guidance: The Department will be meeting with the licensee's radiation safety officer (RSO) and conducting an investigation tomorrow, March 20, 2023.
ENS 5642723 March 2023 15:27:00The following information was received from the Louisiana Department of Environmental Quality (the Department) via email: On March 23, 2023, Nondestructive & Visual Inspection, L.L.C. notified the Department that there was a possible overexposure. The event occurred on March 21, 2023. The industrial radiography crew was working with a SPEC 150 (serial number: 2256) with 80 Ci of Ir-192. The source serial number was EA1810. The industrial radiography crew was working on a pipeline in Red River Parish, Louisiana. The industrial radiographer was not wearing his dosimetry badge, pocket ion chamber, and rate alarm meter when he realized the radiation source had not been cranked back into a shielded condition and was out to the end of the source guide tube with a collimator. The radiographer has had bloodwork done to send to REACT for testing. The calculation for exposure ranged from 1.8 rem to 21 rem. The industrial radiography source is now secured and properly shielded within the SPEC 150 camera." Louisiana Event Report ID No.: LA20230005
ENS 565087 May 2023 11:50:00The following information was provided by the LA Department of Environmental Quality (the Department) via email: (On) May 6, 2023, Shell Chemical LP notified the Department that a fixed gauge has a stuck shutter in the open position. (Gauge information:) Vega Model SHF1 serial number: N0416 with Cs-137 50 mCi, source serial number: N/A. The facility has contacted BBP Sales to come out on Monday, May 8, 2023. The facility is planning to replace the nuclear gauge housing because of corrosion. Event Report ID No.: LA20230008
ENS 5659428 June 2023 11:33:00The following information was provided by the Louisiana Department of Environmental Quality (the Department) via email: On June 28, 2023, Dow Chemical Company notified the Department that a fixed gauge had a stuck shutter in the open position. Vega Model SHF2B serial number: 0368CR with a 500 mCi Cs-137, source serial number: N/A. The facility was using BBP Sales to perform their six-month audit on the gauges. BBP Sales sprayed penetrating lubricant on the shutter which loosened the shutter. They plan to come back to re-spray the shutter and make sure that it functions as designed. ENS 5676830 September 2023 15:18:00The following was received from the Louisiana Department of Environmental Quality (the Department) via email: On September 30, 2023, Acuren Inspection, Inc. notified the Department that an industrial radiography camera failed to retract the source after an exposure. The industrial radiography camera was a Century 330 QSA Cobalt camera. The serial number of the camera is P30078. The radiation source is a Cobalt-60 with an activity strength of 52.2 curie. The source serial number is 59740G. The source was cranked back out the end of the source guide tube with a collimator. The facility where this event occurred is in St. Martin, Louisiana. Only the two man radiography crew was present at the site during the event, which occurred between 0300 (CDT) and 0350. A source retrieval was performed, resulting with the source in a shielded condition in the radiography camera. The individual performing the source retrieval received only 1 millirem. ENS 567712 October 2023 16:43:00The following report was received from the Louisiana Department of Environmental Quality (the Department) via email: On October 2, 2023, Syngenta Crop Protection, LLC notified the Department that a nuclear level gauge shutter was broken. The shutter is stuck open at approximately 40 percent. The gauge manufacturer is Texas Nuclear, Model: 5182, serial number 149. The source is Cs-137 with 50 mCi activity, serial number: J34. BBP Sales has been contacted to come out to the facility to perform repairs on the nuclear gauge. Louisiana Event Report ID No.: LA20230012
ENS 5690622 December 2023 10:56:00The following information was provided by the Louisiana Department of Environmental Quality (DEQ) via email: On December 14, 2023, the licensee was performing a Y-90 procedure. A tubing failure resulted in an incomplete dosing of the patient. All of the unadministered radiopharmaceutical was contained within the administrating device's tubing. There was no spill involved. No effect on the individual was determined. Of the prescribed dose of 105 Gy, only 50.5 Gy was administered. The remainder of the prescribed dose is scheduled to be administered on January 2, 2024. A representative from TheraSphere was in attendance during the procedure and witnessed the tube failure. The TheraSphere representative alerted their colleagues at Boston Scientific. Improvements needed to prevent recurrence: More thorough inspection of device tubing prior to administration. ENS 5692614 January 2024 19:21:00The following information was provided by the Louisiana Department of Environmental Quality (LA DEQ) via email: This medical event was reported (to the LA DEQ) on January 13, 2024, at 2259 (CST). On January 12, 2024, the licensee was performing a Y-90 brachytherapy medical procedure. A tubing failure (catheter) resulted in an incomplete dosing of the patient. The catheter became blocked up with the undelivered radiopharmaceutical Y-90. The Y-90 was contained within the administrating device's tubing. There was no spill involved. The Y-90 being used was TheraSphere from Boston Scientific. Approximately 23 percent of the radiopharmaceutical Y-90 was delivered to the patient. No effect on the individual was determined. The remainder of the prescribed dose will be administered to the patient at a later date. ENS 570054 March 2024 10:25:00The following is a summary of the information provided by the Louisiana Department of Environmental Quality (LDEQ) via email: At 2152 CST on March 3, 2024, a lost source was found along Corporate Blvd., Baton Rouge, LA. The device is a Model IC-51 Calibrator with 1000 mCi of Cs-137 as of August 25, 1980. The label on the device indicates the manufacturer was Gulf Nuclear, Inc., of Webster, Texas. LDEQ took possession of the device on March 4, 2024, and has it at the time of this report. Event Report ID No.: LA20240003 THIS MATERIAL EVENT CONTAINS A 'Less than Cat 3' LEVEL OF RADIOACTIVE MATERIAL Sources that are "Less than IAEA Category 3 sources," are either sources that are very unlikely to cause permanent injury to individuals or contain a very small amount of radioactive material that would not cause any permanent injury. Some of these sources, such as moisture density gauges or thickness gauges that are Category 4, the amount of unshielded radioactive material, if not safely managed or securely protected, could possibly - although it is unlikely - temporarily injure someone who handled it or were otherwise in contact with it, or who were close to it for a period of many weeks. ENS 572199 July 2024 13:27:00The following report was received from the Louisiana Department of Environmental Quality via email: A nuclear gauge was stuck in the open shutter position. This event is considered an equipment failure. The failure occurred while BBP (Bonds Barton Pilcher) tried to remove a screw on the rotor on July 2, 2024. Marathon Petroleum brought in a third party to work on the nuclear gauge. BBP was the third party. Marathon Petroleum plans on having BBP replace the nuclear gauge with another one as soon as possible. Device information is as follows: Model: Ohmart SH-F1A Serial number: 13524941 Isotope: Cs-137 with an activity of 15 mCi Source serial number: 65511 The device was acquired in June 2006. The following additional information was obtained from the licensee in accordance with Headquarters Operations Officers Report Guidance: There were no reports of exposures and the licensee has taken measures to prevent future potential exposures.
