SUPPLE! ENTAL INFOPJ!ATION FCR LER NP-33-78-99 DATE OF EVENT: July 24, 1978 FACILITY: Davis-Besse Unit 1 IDENTIFICATION OF OCCURRENCE: Floor Drain Flapper Valve Removed From Drain Conditions Prior to Occurrence:

The unit was in Mode 2, with Power (MRT) = 0, and Load QNS) = 0.

Description of Occurrence: During a routine operator visual inspection on July 24, 1978, at 1130 hours0.0131 days <br />0.314 hours <br />0.00187 weeks <br />4.29965e-4 months <br />, a floor drain flapper valve was discovered to be out of the floor drain in Mechanical Penetration Room No. 4.

These floor drain flappers are to be removed with-toremaininthefloorgrainafterMode4isenteredandarenot out permission of the hift Foreman. Also, the personnel involved in removal of the valve should not leav phe area until the valve is replaced.

This is required by Special Order No. 83 June 30, 1977).

o Designation of Apoarent Cause of Occurrence:

The floor drain flapper valve was s removed at some time during the outage to place a drain hose in the drain and was never replaced after the work was conpleted.

Analysis of Occurrence _: There was no danger to the health and safety of the public or to unit personnel. The floor drain flapper valve being out of the flocr drain is a violation of administrative controls.

It is unknown at present whether this actually violates shield building integrity.

Therefore, a test is going to be per-formed to determine if the Emergency Ventilation System can meet the drawdown require-ments with a floor drain flapper valve renoved. This will determine if this occur-rence violates shield buildicg integrity.

This test will be performed as soon as Facility Change Request 78-383 is approved.

Corrective Action

The floor drain flapper valve was replaced on July 24,1978, and the drains are going to be identified further with ' plastic tags stating, "Do NOT RDiOVE WITHOUT FIRST NOTIFYING TIIE SHIFT FOREMAN".

Failure Data: Shield building integrity has previcusly been violated (Licensee Event Reports NP-33-77-33, NP-33-77-34 and NP-33-77-47), but those occurrences were not due to floor drain flapper valves being out of place, but were caused by shield building doors being open, f))
