Dear Mr. Whitten:

This information is provided to supplement the application for a special nuclear liceg g submitted on January 10, 1986. All activities associated with the Pu will be conducted under the Supervision of the RadiationSafetyCommitteeestablishedforLggsianaStateLicense LA0001-L01. Specific points related to the Pu are:

The sources shall be leak tested every three months.

The source storage room (location of the Pu sources) shall be surveyed for gamma and neutron levels and radioactive contamination every three months.

Records of leak test, radiation surveys, personnel monitoring, radiation training and minutes of Radiation Safety Committee Meetings are and will be maintained by the Radiation Safety Officer in the Radiation Safety Office.

The current Pu sources possessed under LA0001-L01 are stored in a neutron howitzer model number NR-2M3, manufactured by U.S.NuclearCorp.,Burbank,Californig39and a parrifin filled drum. The additional 10 curie Pu source, which will be obtained once the special nuclear material license is issued will be stored in a neutron howitzer model number NR-2M3, manufactured by U.S. Nuclear Corp., Burbank, California.

Attached is a drawing of the source storage area located in the basement of the Nuclear Science Center on the campus of Louisiana State University at Baton Rouge, Louisiana. The building is equipted with a security system tied into the campus police. This system is active at night and on weekends, i.e., when the building is not occupied. Keys to the outside door to the basement are controlled by the Nuclear Science Center Director and the Radiation Safety Office. At this time, five keys are issued. The door to the source storage area is self locking and the keys are controlled by the Radiation Safety Office. At this time four keys are issued.
