IRF10 INSERVICE INSPECTION

SUMMARY

REPORT SECOND INTERVAL, SECOND PERIOD, FIRST OUTAGE 1.0 INTRODUCUTON 1.1 PURPOSE The purpose of this summary report is to provide information relating to the inservice inspection (ISI) activities performed during the tenth refueling outage at the Comanche Peak Steam Electric Station (CPSES) Unit 1. This report documents compliance with I 0CFR50.55(a), Technical Specifications, and ASME Section XI, 1986 Edition, no Addenda (the Code), for examinations and tests. This report also documents compliance with the Texas Department of Licensing and Regulation, Boiler Division, Boiler Law & Rules and Regulations. 1.2 SCOPE This summary report addresses those examinations and tests of Code Class 1, 2 and 3 systems as stipulated in the CPSES Unit I - Second Interval - ISI Program Plan, Revision 2 (the Plan). The NIS-1 Form is included in Appendix A. A list of the NIS-2 Forms documenting Code Repairs/Replacement activities that have occurred since issuing the previous NIS-1 Form for lRF09 are contained in Appendix D. The period of time covered for the Code Repairs/Replacement activities is from February 1, 2003 to July 31, 2004. 1.3 OUTAGE, PERIOD AND INTERVAL START AND END DATES This refueling outage was the first outage in the second inspection period of the second inservice inspection interval. The ISI examinations for this Report were performed between March 10, 2004 and June 24, 2004. IRF10 outage ISI exams were performed between March 29,2004 and April 27, 2004. The Inspection Period began in August 2003 and will end in August 2007, after the completion of the twelfth refueling outage. The Interval began on August 13, 2000 and is scheduled to end on or about August 13, 2010. 1.4 AUTHORIZED NUCLEAR INSERVICE INSPECTOR Tests, examinations, repairs and/or replacements were witnessed or verified by Mr. Joe C. Hair, Authorized Nuclear Inservice Inspector for Hartford Steam Boiler-Connecticut (HSB-CT), as indicated by his signature on the examination data sheets. The HSB Regional Office is located at 200 Ashford Center North, Suite 300, Atlanta, Georgia 30338.

SUMMARY

OF EXAMINATIONS AND TESTS 2.1 PIPING AND COMPONENTS Examinations were conducted utilizing volumetric, surface and visual techniques in accordance with the requirements of the Code. Appendix B provides a summary of the examinations performed during this outage. Where a particular examination requirement of the Code is determined to be impractical, a request for relief from the requirement is submitted in accordance with the provisions of 1 OCFR50.55a(g)(6)(I). As permitted by Code Case N-460, relief is not requested when a reduction in examination coverage on a Code Class 1 or 2 weld is less than 10%. The relief request number is identified in the remarks column of the summary for each examination area requiring relief. Appendix C is blank as no relief requests were generated as a result of this outage.

Volumetric Examinations Thirteen (13) ultrasonic examinations were completed. There were no unacceptable indications identified.

b.

Surface Examinations Eleven (11) liquid penetrant examinations and four (4) magnetic particle examinations were completed. There were no unacceptable indications identified.

c.

Visual Examinations One hundred four (104) visual examinations of components and their supports were completed. One recordable indication was identified for a loose clamp on a pipe support and was documented as part of the corrective action process in Smart Form (SMF) -2004-001225. Work Order (W.O.) 4-04-154320-00 resolved the condition, with the clamp assembly satisfactorily re-examined. Another recordable indication involved an apparent loose strut on each of two pipe supports and was documented in SMF-2004-001135. It was reviewed and later evaluated as acceptable in the SMF. White residue was identified on 3 of 8 threads at a flange connection and was documented in SMF 2004-001243. The threads were cleaned under W.O. 4-04-1543345-00, with a satisfactory re-examination performed. Non-recordable indications (NRI) are found on pages 10, 22, and 24 of Appendix B. There were no conditions identified that adversely affected the ability of a component or support to perform its intended function. There were no other unacceptable conditions identified.

2.2 PRESSURE TESTS System leakage, functional and inservice tests were performed during the period covered by this report. These tests were conducted under the following pressure test packages: l-AF-2-2-1 I -MS-2-2-la I -RH-2-2-1 I-RC-2-2-l a I -AF-2-2-2 I-MS-2-2-lb 1-RH-2-2-2 I-RC-2-2-lb I -AF-2-2-3 I -MS-2-2-2 I -RH-2-2-3 I -RC-2-2-1 c 1 -AF-2-2-4 1 -RC-2-2-1 d l-AF-2-2-5 1 -RC-2-2-1 e All areas of leakage, if any, were evaluated for corrective measures as described in IWA-5250. SMF2004-001052 documents where small leakage of 15 drops/min was observed at manway cover 2B of Steam Generator, TBX-RCPCSG-02, during the system pressure test of package I -MS-2-2-2. The gasket seating surface of the manway was repaired and a subsequent VT-2 test was performed satisfactorily. All of the other, above identified pressure test packages, have been successfully completed. 2.3 CORROSION MONITORING The Corrosion Monitoring Program examined 116 individual components in accordance with STA-730 during IRFIO. These components consisted of piping elbows, tees, reducers and expanders, straight pipe (primarily downstream of valves and orifices), and nozzles. The following provides a listing of the components per system that were examined during IRFIO: 4 - Component Cooling Water 1 - Condensate 5 - Extraction Steam 44 - Feedwater 43 - Heater Drain 10 - Main Steam 2 - Steam Generator Blowdown 4 - Service Water 3 - Auxiliary Steam Initial screening of the ultrasonic thickness measurement data indicated that there was no evidence of significant wear (Flow Accelerated Corrosion) in any of the components inspected during 1RF 10. The results of the ultrasonic thickness examinations and subsequent analyses of the data wvill be found in the IRFI 0 Corrosion Monitoring Report.
