Cont ra ry to t he above , s teps 4.5, 8.5.2 a nd 8.6 of Admi ni strative Procedure 0103.4, "In Plant Equipment Clearance Orders" were not f ully impl emented for equipment clearance order (EC0) 7-155 in effect from 7/19/82 to 7/23/82. A danger tag was improperly pl aced , the EC0 form wa s not completed, and the clearance wa s not properly removed f rom val ve MOV-878B i n the sa fety i njection s ys t em .
