Procedure A. P. 0103.4 speci fies the method fo r placing and remo vi ng tags for administrative control of equipment.

Cont ra ry to t he above, s teps 4.5, 8.5.2 a nd 8.6 of Admi ni strative Procedure 0103.4, "In Plant Equipment Clearance Orders" were not f ully impl emented for equipment clearance order (EC0) 7-155 in effect from 7/19/82 to 7/23/82.

A danger tag was improperly pl aced, the EC0 form wa s not completed, and the clearance wa s not properly removed f rom val ve MOV-878B i n the sa fety i njection s ys t em.

FPL concurs with the finding.

The reason for this finding was that the procedure fo r i n-pl ant clearances did not clearly define where' tags were to be placed nor did the procedure include a wo rka ble mechani sm fo r tempo rary rel eases of clea rances.

As corrective action, the danger tag was removed from MOV-8788 and the operator wa s counselled on the need fo r
