On Hay 1994, Coltec Industries. Fairbanks Horse Engine Division (FHED), notified Baltimore Gas end Electric Co.(BG&E) vfa Engineering Report No.: R-5.08-4306 that the Fuel Injection Erosion Sleeves (part number 03730049) experienced accelerated wear during a 2000 hour endurance test. The failure of the sleeves required engine shut down at the 1313 hour mark.

FHED is currently pursuing an evaluation pursuant to the requirements of Part 21 to determine whether This situation could potentially affect all Nuclear Utilities who operate O.P.38TD8-1/8 Emergency Diesel Gen-Sets. FHED will notify all affected engine owners via letter from our Parts and Sales Harketing Group.
