Four (4) of the five (5) WOL flaws are located outside of the WOL examination volume. All five (5)WOL flaws are acceptable in accordance with ASME Section XI, IWB-3514 and the additional requirements of Relief Request N-14-CMP-001.

The WBM planar flaw is a pre-existing flaw outside the examination volume and therefore is acceptable to the requirements of the NAPS Relief Request See the IWB-3514 flaw characterization and evaluation on the following pages.Examination Coverage: Examinations achieved 90.5% coverage of the required volume of the Weld Overlay material examination volume for Laminar Flaws.Examinations achieved 97.7% coverage of the required volume of the Weld Overlay material examination volume for Planar Flaws.Examinations achieved 99.6% coverage of the required volume of the PSI -ISI examination volume.Examinations achieved 100% coverage of the susceptible material within the examination PSI -ISI examination volume. The applicable Calibration Data Sheets are listed below: NAPS SGA / C -WOL -0 AXIAL; NAPS SGA / C -WOL -180 AXIAL NAPS SGA / C -WOL -0 CIRC; NAPS SGA I C -WOL -180 CIRC Analysis: Reviewer: Level: Rr Date: Oj/2s//z Level: Date: Page of 11 Dominion: ANII: Date: Date: -"

All six flaws are acceptable in accordance with ASME Section XI, IWB-3514 and the additional requirements of Relief Request N-14-CMP-001.

See the IWB-3514 flaw characterization and evaluation on the following pages. In addition, five (5) axial service-induced planar flaws were detected in the original weld and butter (WMB)of the component Two of these WBM axial planar flaws extend into the PSI / ISI examination volume. These five (5) axial service-induced planar flaws are acceptable, as indicated in SIA Calculation Package File No.: 1100226.310 "North Anna Steam Generator Hot Leg Inlet Nozzle Weld Overlay Sizing Drawing".Examination Coverage: Examinations achieved 90.1% coverage of the required volume of the Weld Overlay material examination volume for Laminar Flaws.Examinations achieved 95.9% coverage of the required volume of the Weld Overlay material examination volume for Planar Flaws.Examinations achieved 99.7% coverage of the required volume of the PSI -ISI examination volume.Examinations achieved 100% coverage of the susceptible material within the examination PSI -ISI examination volume. The applicable Calibration Data Sheets are listed below.NAPS SGB -WOL -0 AXIAL; NAPS SGB -WOL -180 AXIAL NAPS SGB -WOL -0 CIRC; NAPS SGB -WOL -180 CIRC Analysis: Reviewer: Level: lVDate: a't'Level: A / Date: Dominion: , 14 Date: Date: J J f H J I Page 1 of 30 Structural Integrity Associates PHASED ARRAY ULTRASONIC EXAMINATION RECORD i Customer.
