Dear Mr. Davis:
The following information is submitted as requested in DRO Balletin s
No. 72-3, "Limitorque Valve Operator Failures", dated December 4,1972.
y/ Our investigation reveals that there are seventy-five (75) operators which are Limitorque models SMB-00 or SMB-000 and may have been manufactured between 1969 and mid-1971. These operators are associated with valves which are installed, or scheduled to be installed, in the following systems: In-termediate cooling water, circulating water, service water, reactor coolant, decay heat, emergency feedwater, main steam, makeup, sampling, reactor build-ing spray, core flood, and chemical addition.
We have corresponded with the Limitorque Corporation through our Architect Engineer and NSSS supplier and have received their cooperation in identifying operators that may contain defective torque switches and supplying the required replacement switches. In addition, Limitorque has provided a field representa-tive to instruct AP&L personnel involved in replacement activity.
