On January 18, 2024, the licensee's radiation safety officer (RSO) was completing shutter checks and leak tests on a Berthold fixed gauge, model number: LB-300 W, serial number 1744-11-14 containing sealed source number P-2608-100 with 8 millicuries of Co-60.
