ISSUANCE OF FINAL SIGNIFICANCE DETERMINATION AND NOTICE OF VIOLATION This is to inform the Commission that a Notice of Violation (NOV) associated with a White Significance Determination Process finding will be issued on or about August 8, 2014, to Duke Energy Carolinas, LLC (licensee) as a result of an inspection at Oconee Nuclear Station, Unit 1.
ISSUANCE OF FINAL SIGNIFICANCE DETERMINATION AND NOTICE OF VIOLATION This is to inform the Commission that a Notice of Violation (NOV) associated with a White Significance Determination Process finding will be issued on or about August 8, 2014, to Duke Energy Carolinas, LLC (licensee) as a result of an inspection at Oconee Nuclear Station, Unit 1.
The White finding, an issue of low to moderate safety significance, will result in additional NRC inspection and potentially other NRC action.

The White finding involved the failure to identify and correct a significant condition adverse to quality involving a crack in a weld located in the Unit 1 High Pressure Injection (HPI) system. In 2004, a procedure was developed for augmented in-service inspection program ultrasonic examinations, which effectively removed reasonable assurance that HPI nozzle component weld cracking would be identified and corrected. Consequently, the crack continued to propagate resulting in reactor coolant system pressure boundary leakage that required Unit 1 be shut down until the weld was repaired. A NOV is included because of the licensee's failure to establish measures to identify and correct a significant condition adverse to quality in accordance with 10 CFR 50, Appendix B, Criterion XVI, "Corrective Action".
