This;inspectionwasconductedto*rev.ibwthe. circumstance'sfollowingareactor

trip on: January 20, 1989, which led to the event where reactor coolant

.

backleakage occurred through a'high pressure safety injection check valve which

failed to. reseat. .Thisfresulted'in a high temperature condition on,a crossover-

line in'a piping penetration room outside-of containment. The enclosed copy of.

1the AIT report describes the-areas examined during this review. Within these

areas, the inspection consisted of selective examination of procedures and'

l representative records, interviews with personnel, and observations by the NRC

inspectors.
